Safety Feedback Notices review and analysis

SAFETY FEEDBACK NOTICES
SINCE JULY 2005 UNTIL TODAY

We received…
• 20 safety feedback notices coming from TOTAL E&P,
• 05 safety feedback notices coming from external sources.

We also issued…
• 01 safety feedback notice regarding electrical hazards (electrical anomaly report regarding contractors),
• 01 safety feedback notice regarding the result of a truck driver accident (chain binder handler in the face),
• 01 safety feedback notice regarding the use of cell phones in hydrocarbon facilities,
• 01 safety feedback notice regarding last finger/hand accidents on our block (2 x hand injuries/HTC).

EXAMPLE OF SAFETY FEEDBACK NOTICES RECEIVED (AND HAVING A PARTICULAR INTEREST ON SITE)

• Welder crushed (fatality, TOTAL E&P),


• Lifting Incident in a shipyard in Malaysia,
• Cell phones and petrol station (Petro SA),
• Isolation of energy sources (TOTAL E&P),
• Stop work for safety (TOTAL E&P),
• Crane incident (Shell),
• Car rollovers (TOTAL E&P RED ALERT),
• Grinder accident (AMEC-Spie),
• Temporary pipe supports (ESSO)…

WHAT WE DO…

• Analyze by concerned Superintendents and dispatching,


• Comment during our HSE Trade Committee meetings,
• Display on all our site HSE Boards (and any contractor Notice Board),
• Organize special safety talks focused on the subject (with TEP-Y trades and at each contractor camp),
• Immediately auditing our installations and contractors,
• Implementation of corrective action plan if necessary.

WHY…

• To share experience with all personnel (TEP-Y & contractors),


• To share experience with other TOTAL E&P Subsidiaries and external companies,
• To avoid any replication of the incident/accident/fatality within our subsidiary.

SOME EXAMPLE…

• Grinder accident “draw to danger” (received from AMEC-Spie)


 Diffusion on all safety boards,
 Diffusion on all contractors’ restaurants,
 Safety Talks conducted on all contractors’ camps,
 HSE Trade meeting conducted with all Maintenance and Construction personnel,
 Onsite (on all construction areas, Kharir + Atuf) demonstration by our safety Officer of what can
happen when using a grinder while wearing an “Emama”, ID card lanyard, or any long jewelry…

 Result:
– Satisfactory safety consciousness during the safety talks,
– Medium “on the field” results during site reviews (Supervisor’s safety improvement/concern required)

• Car rollover (RED ALERT received from TOTAL E&P)


 Diffusion on all Safety Boards,
 Mentioned during the monthly Site HSE Committee meeting to the Trade Leaders,
 Memo issued to enforce the VDO system (& results),
 Setup systematic driver assessment to all new comer on site (including visitors wishing to drive),
 Speed controls on the block.

 Result:
– Satisfactory for all TEP-Y drivers – Medium-to-Nil effect on contractors,
– Stat’s results difficult to obtain in time = limited effect

SOME EXAMPLE (suite)…

• “Electricity can kill you” (Edited by TEP-Y)


 Diffusion on all Safety Boards,
 Immediate HSE review (electrical audit) on all contractor’s camps and workshops,
 Edition of anomaly (PS-AR-67/05 Masarco) / incident (PS-IR-69 Hawk) reports,
  destruction of all unsafe electrical cables, plugs, …

 Result:
– HTC camp electricity is still highly dangerous,
– HAWK portable electrical equipment are still (randomly) highly dangerous, we still continue to remove
some dangerous electrical equipment from their work locations …
– Nowhere (TEP-Y new cabins, Masarco, Precision, …) it is a standard to use Earth Leakage Circuit
Protection.
