Indonesia adalah negara dengan endemisitas tinggi Hepatitis B, terbesar kedua di South
East Asian Region (SEAR) setelah Myanmar. Dari 100 penduduk Indonesia, 10
diantaranya terinfeksi Hepatitis B atau C. Artinya 28 juta penduduk Indonesia terinfeksi
Hepatitis B atau C, 14 juta diantaranya berpotensi kronis.

Morbiditas Mortalitas :
• Infeksi akut hepatitis B dapat menyebabkan
kematian karena nekrosis hepatik fulminan.
• Infeksi kronik virus hepatitis B merupakan
salah satu penyebab kematian yang
berhubungan dengan kegagalan hepar,
sirosis dan kanker hepar (hepatoma).

In addition, high maternal HBV DNA and

treatment with vaccine alone were significant
factors for overt HBV infection in babies.
The current practice of administration of vaccine
with HBIG at birth to babies born of HBsAg-
positive mothers is not effective in preventing
occult HBV infection in babies, which may be up
to 40%.
Because the most important risk factors for
mother-to-baby transmission of HBV infection
are the replicative status and high HBV DNA
level in mothers; it will be worthwhile
investigating the role of antivirals and HBIG
administration during pregnancy to prevent
mother-to-child transmission of HBV infection.

• Pemberian HBIG dan vaksin Hepatitis B pada bayi

yang lahir dari ibu dengan Hepatitis B, ternyata
tidak cukup melindungi bayi dari kemungkinan
menderita infeksi kronik Hepatitis B.

• Hal paling penting dalam transmisi intrauterin Hep

B adalah status replikasi virus dan kadar HBV DNA
pada maternal.

Hepatitis B Surface Antigen (HBsAg):

Petanda serologi dari surface of HBV, pada fase akut atau kronik.
HBsAg positif mengindikasikan seseorang terinfeksi virus
hepatitis B.

Hepatitis B Surface Antibody (anti-HBs):

anti-HBs mengindikasikan imunitas dan kesembuhan dari infeksi
HBV. Anti-HBs juga dibuat oleh sistem imun tubuh sebagai respon
vaksinasi yang sukses.

Total Hepatitis B Core Antibody (anti-HBc):

Muncul saat onset akut hepatitis B dan menetap selama hidup.
Anti-HBc mengindikasika infeksi lampau atau yang sedang
berlangsung tanpa waktu yang jelas.

IgM Antibody to Hepatits B Core Antigen (IgM anti-HBc):

Antibodi ini muncul saat infeksi HBV akut atau terbaru dan
menetap sekitar 6 bulan.

Lamivudine Therapy During Pregnancy to Prevent Perinatal

Transmission of HBV Infection

• Highly Viraemic (HBV-DNA > 1.2 x 10(9) geq/mL)

Mothers Treated With 150 mg of Lamivudine Daily Since 34 Wks
of Gestation (n=8)

12.5% HBsAg and HBV-DNA positive until 12 months

• Mothers not treated (n=24), 28% perinatal transmission occur

• All children received passive-active immunization at birth .

• Rute persalinan untuk mengurangi risiko transmisi HBV

masih dipertanyakan terkait bukti yang belum sahih.

• Beberapa penelitian terdahulu : laju transmisi HBV pada

bayi yang lahir melalui operasi SC, tidak menunjukkan
perbedaan bermakna dengan pervaginam.

• SC tidak disarankan sebagai indikasi menurunkan

transmisi vertikal HVB.

• Laju transmisi HBV 4,37% (223 of 5105) pada
persalian dengan SC dan 9,31% (447 of 4801)
pada pervaginam.
• Risiko relativ (RR) 0,51 (95%CI: 0.44–0.60, P <
0.001), mengindikasikan penurunan transmisi
vertikal yang bermakna pada persalinan SC
dibandingkan pervaginam.

Rekomendasi perlindungan Bayi Baru lahir :

• Vaksin Hep B 3 kali
• HBIG 100 IU IM
< 12 jam setelah bayi lahir

• Status hepatitis bayi harus diperiksa

setelah vaksinasi selesai
