HISTORIA CLÍNICA
RESUMEN DE
Mujer de 26 años con cuadros repetidos de cefalea desde la menarquia que describe
como dolor pulsátil hemicraneal (más frecuente en el lado derecho) que venían
precedidas, durante 20 minutos, de “visión de puntos brillantes” con borrosidad visual por un
hemicampo visual (que podía cambiar de un episodio a otro).
La cefalea se asocia a intensa sensibilidad a la luz y olores fuertes , con malestar general,
náuseas e intensificación del dolor con los movimientos de la cabeza o ante el menor
ejercicio físico. La duración es variable, de cuatro horas a dos días, permitiéndole el
descanso nocturno, pero le imposibilita ir al trabajo. La frecuencia es de dos a tres episodios
por mes, coincidiendo la mayoría de las veces alrededor de la menstruación.
Una vez remitido el dolor, sufre un intenso cansancio y somnolencia durante todo un día. En
otras ocasiones la cefalea no viene precedida de los síntomas visuales y, en esos casos, el
dolor es menos intenso y de menor duración. Mejora con la toma de 1.100 mg de
naproxeno más 10 mg de metoclopramida.

frecuencia de 3 veces al día
