    Title/ Name and Type of Project
    Brief Description: Includes objectives of the Proposed Project
    What is the project about?
    Project Activities / Timelines : (Identify activities and tentative work plan)
    Partner Organizations: Include roles of partners
    Rationale (Establish need for the program based on CDRA Results/ other assessment)
    Estimated Number of Beneficiaries If possible provide targets
    Estimated Cost Include topline details (Materials, Human Resources, Capital Outlay)
    Project Location In what barangays?
    Target Outputs / Desired Outcome Relate outcomes to development indicators/ national plans
    Risk Analysis/ Plan for Mitigation What factors can affect implementation?
