Engagement Feedback Form
Completed for every engagement over 120 hours
For long-term engagements, a form is required, at a minimum, every six months
Mid-Year Feedback Form
Primary outcome is an actionable counseling message
Counselor will compile and synthesize feedback; Counseling Tree Leader will finalize and approve
message
Year-End Feedback Form
Self-assessment completed by Counselee
Feedback section completed by Counselor
Counselor presents to Counseling Tree leader, who will represent Counselee during Year-End
Feedback sessions
Leadership Team will normalize appraisals and assign rating based on performance against Career
Development Guidelines, relative to peers
