05/17/09 1:14 pm. 911 alert came in to GEOS Alliance. Lat. 45.3719, Long. 121.6985. SPOT ESN: 0-7352032

05/17/09 1:16pm. GEOS called victims primary and secondary emergency contact. No answers.

05/17/09 1:16pm. GEOS called Oregon Dept. of Emergency Management in Salem.


Informed OEM of the 911 alert and gave them coordinates of the incident/victim.

05/17/09 1:50pm. GEOS called Oregon OEM to give update on coordinates. OEM told GEOS rescue is being transferred to
local responding agency - Clackamas County sheriff 503-655-8211.

05/17/09 3:05pm. GEOS spoke with Clackamas County Sheriff's Office. CCS states that Portland Mountain Rescue Team
is on location with the patient, who has critical injuries: 2 broken wrists, broken leg, major facial and head trauma. Patient is
unresponsive and having problems breathing.

05/17/09 5:30pm. GEOS received call from CCS, Deputy M. Styner, He requested updated GPS from the unit. Also states that
Coast Guard Helicopter is not available. Will wait for Blackhawk from Oregon National Guard at PDX Airport.

05/17/09 7:00pm. Blackhawk helicopter arrives on scene of victim. GEOS called Clackamas County Dispatch and requests
that client power the unit off. CCD stated they will being taking the patient soon to OHSU Hospital in Portland.

Victim slipped near the summit, and fell over 400,


sustaining severe injuries at about 10,600'.
