form. Targets shall be reflected.

your own indicators . Just
copy the indicators as
reflected in the given
template.

Example:
Provided assistance to 100% of the
schools in preparing School
Improvement Plan and LC
Improvement Plan quarterly.
ACTUAL Result: Provided technical
assistance to 18 out of 20 schools or
90%.

Compare your results versus your
target. Then refer to the
indicators.
Since your achievement is 90%
only, your rating will be 4 in
terms of quality if all required
MOVs were realized. Quality of
work will also be measured .

How to compute

1. Determine the rating per objective as to


effectiveness, efficiency or timeliness.
2. Get the average of rating per objective
then multiply based on the assigned
weight. Do this procedure for the rest of
the objectives under the same KRA.
3. Add all scores to get the sub-total per KRA
4. Add all sub-totals to get the over-all rating.
5. Refer to the scales to determine the
descriptive rating

Development Planning
1. Review the accomplishments per objective.
All objectives with high scores are the
strengths.
2. The objectives which were not realized as
reflected by low ratings in other objectives
are your development needs.
3. Based on the identified needs, the rater and
the ratee will agree on the developmental
interventions to be done to improve the
performance for the next rating period. This
development plan shall be attached to the
next OPCRF/IPCRF which will serve as guide
of both rater and ratee in tracking the
performance.
