A source or a situation with a potential for

harm to humans, property and damage of
environment or a combination of these.
Danger
Relative exposure to hazard.
Risk
A combination of likelihood of occurrence
and severity of injury or damage.

HAZARDS
ELEMENT I

HIRARC
Module 2: Defining Hazard and Risk
Every job has hazards, risks and consequences

Responsibility for safety

lies with all people within
our organization. We have
a personal responsibility
and accountability to work
safety, to care for those
around us and to take the
necessary
action
to
prevent injuries from
occurring.

What is a hazard?

What Types and Examples of Hazards Might We Encounter?

To achieve our goals and objectives in the oil-field service business we must do the
following three things well:
1. Identify risks
2. Control hazards
3. Manage change effectively
We will begin with a focus on hazards and hazard control. A hazard can cause
harm or adverse effects to individuals as health effects. Hazard control is achieved
by understanding and reviewing the controls in place to manage the risks.

Hazard identification and Reporting
Hazard identification Methods
Observation activity ongoing or walking
through the worksite .. observes unsafe act or
condition
Inspections Routine inspections e.g. fire
extinguishers, engines,
Pre-use checks quick visual checks before
using equipment or accessories lifting
accessories,
Checklists - prompt cards, LOTO checklists,
pre-start checks

How To Assess Risk
1) Look for the Hazards
2) Decide who might be harmed & how
3) Evaluate the risk and check what is
done to prevent it from happening

4) Record finding

5) Review assessment and revise it if

necessary

HIRARC
Quantitative Risk Assessment
In

cases where hazards are numerous

and complex
eg; Chemical process plant
Should have Job Safety Analysis (JSA)
. describe job in less than 10 steps
. List things that can go wrong
- eg; Changing a Car Wheel

HIRARC
JOB SAFETY ANALYIS (JSA, JHA)
Three step process
Identify each step of a task or procedure by
breaking it down into its component parts.
Identify the Hazards and Potential for
Incidents associated with each key step.
Devise safety controls to mitigate those
hazards and eliminate or reduce the
potential of occurrence.
