THE DECISION TO HAUL INDEX #02 WAS IN
ACCORDANCE WITH MRT3 OPERATING PROCEDURES.

INDEX #02 STALLED DUE TO OCS UNDERVOLTAGE AND FAILED TO REGAIN
TRACTION DESPITE INTERVENTIONS BY THE TRAIN DRIVER FOR ABOUT 12
MINUTES WITH NO ASSISTANCE FROM THE MAINTENANCE PROVIDER AS
THERE WAS NO EMERGENCY REPAIR UNIT PERSONNEL IN THE AREA DURING
THE ENTIRE COURSE OF THE TRAIN PROBLEM.

THE DECISION OF THE CONTROL CENTER
DUTY MANAGER NOT TO UNLOAD PASSENGERS OF
INDEX #02 IN MAGALLANES STATION WAS BASED ON
HIS BEST JUDGEMENT AT THAT TIME
BUT WAS NOT IN ACCORD WITH RS 110A, RS 110B AND
OFFICE ORDER NO. 059
FINDINGS # 3.

THE COUPLING PROCEDURE PERFORMED BY BOTH TRAIN DRIVERS WAS NOT IN
ACCORDANCE WITH ROLLING STOCK OPERATION PROCEDURES,
PARTICULARLY RS105A, RS 110A & RS 110B
FINDINGS # 4

THE CONTROL CENTER SUPERVISOR ON DUTY FAILED
TO EXERT EXTRA EFFORTS TO GUIDE BOTH TRAIN
DRIVERS IN THE PERFORMANCE OF THE COUPLING
PROCEDURES SO AS TO ENSURE THAT BOTH TRAINS
WERE PROPERLY COUPLED BEFORE HAULING/TOWING
FINDINGS # 5

THE MECHANICAL COUPLERS OF CAR #33 AND CAR #08
WERE FOUND TO BE NOT DEFECTIVE

BOTH COUPLERS WERE TESTED AFTER THE ACCIDENT BY BOTH THE
MAINTENANCE PROVIDER AND DOTC-MRT3 REPRESENTATIVES AND FOUND
THE SAME TO BE IN GOOD WORKING CONDITION
IMMEDIATE CHANGES IN MRT3 OPERATIONS

All measurements are on a per 20-min
period. Any fraction thereof shall be
computed as a full 20min. delay.

operated as per approved timetable,
which, fault is caused by failure of the
related systems affecting revenue service
(stoppage of operation, provisional service,
etc) including the Traction Power System,
OCS, Tracks, Signaling and Permanent
Ways

Penalty amount is 50% of Reference
Penalty (P40,000.00) per car per hour.

Note: Any fraction of less than an hour
shall be pro-rated on a per hour basis.

A. Elevator- 23 out of 34 are operational as of 20 August 2014
B. Escalator- 37 out of 46 are operational as of 20 August 2014
2. Rolling Stock- 57 out of 73 LRV are operational as of 20 August 2014
-LRV # 73,08 and 33 awaiting safety certificate from Global-APT JV
3. AFCS
A. Automatic Gates- 158 out of 171 are operational as of 20 Aug 2014
B. Ticket Issuing Machine- All Ticket Issuing Machine are not operational
as of 20 Aug 2014.
C. Analyzer Dispenser- 81 out of 81 are operational
D. Station Computer System- 13 out of 13 are operational
E. Central Computer System- 1 out of 2 are operational
F. Encoder Sorter 2 out of 3 are operational
4. Signaling
MAN900- 2 out of 3 interlocking areas are operational as of 02 July 2014
5. Communication
Requires Tuning Software for calibration of frequency of the repeaters
6. Depot Equipment
Wheel Truing Machine- Non operational since 05 June 2014
7. Tracks- 2.5 out of 29 spare rails available as of 20 August 2014


Certificate of the services rendered and
delivered

DOTC-MRT3 has not recommended the issuance of a certificate of services
rendered and delivered considering the works that have not been repaired.
The said Certificate is an attachment to the Maintenance Providers Billing
Claims 7,8,9 and 10 corresponding to the billing period 05 March 04 July
2014.
