Professional/ Scientific organizations are national bodies or professional practitioners in a specific field or science, e.g., Philippine Inventors Society, Philippine Society for Mechanical Engineers.

3.5.1 For every year of participation in service-oriented projects in the community . . . . . 1.00 N.B. These are activities related to participation in community projects on top of official duties and are voluntary in nature, with no additional compensation received. One year is operationalized to be equivalent to at least 251 days, not to exceed 12 months per program per clientele.

Examples are voluntary services in scouting, cooperatives, Red Cross, Kabataang Barangay, etc.
Activities undertaken as officer/ member of any professional , social, educational, or other similar organizations which have already been credited in this instrument shall no longer be considered.

Things to check for CCE and QCE at the Local/ College Level

The Personal Data Sheet (PDS) must have the signature of the faculty in all pages. The PDS must be notarized (subscribed and sworn to ). All documents for CCE and QCE must be supported with original and Xeroxed copies. All documents shall be stamped with the words True Copy and Relevant at the University Level. Faculty with incomplete QCE due to scholarship or study leave should attach the scholarship contract and Board Resolution with the university seal and authenticated by the Board Secretary. All CCE and QCE folders with their supporting documents must be accompanied by the required forms for Evaluation and Summary of Points. The CCE and QCE documents must be placed in separate folders and separate envelopes. Follow color coding. Make a list of faculty with CCE and QCE for submission to the University Level.

REMINDERS IN FILLING UP FORMS FOR THE NBC 461 5TH CYCLE (QCE)

1. The right side of the FOURTEEN (14) top sheets is reserved for the University level signatories. 2. Conforme of the Ratee should be at the center only. 3. If your instrument is printed/ mimeographed back to back, put fastener at the center for easy checking. 4. Replace evaluation with discrepancies in: a. Color and impression of ink. Even if color is the same, the ballpen point may not be the same. b. Penmanship in the lower portion of the instrument c. Strokes of ratings (the encircled portion)

5. All evaluations must have complete specific dates, not just the month and year. 6. Arrange from June 2007- to March 2010 not vice versa. The order of evaluations should start from students, then peers, self and supervisor. 7. Supervisor of faculty should be the Department Head at the time of the rating period. For the Dean, it should be the VPAA. 8. Check the Rating Period in the top sheets. It should be semestral rating period. 9. Rating period in the Consolidated Summary of Ratings for the six semesters is July 01, 2007 June 30, 2010. 10. Avoid erasures. 11. Follow strictly the deadline for submission of faculty documents for local level and university level committees.
