40.1 Equivalents Record Forms (ERFs) PART IV, Agency Specific Expenditure Items National Budget Memo. No. 112 s. 2011
Lump-sum appropriations for the reclassification of teaching and related teaching positions in DepEd based on ERFs of incumbents approved as of April 30, 2012 shall be provided on the basis of actual requirements.
Appropriations for ERFs shall not exceed the FY 2012 budget.
1. Updated SERVICE RECORD. The applicant must have rendered at least one (1) year continuous service.
4. 2 copies Sworn Statement certifying the authenticity of Transcript of Records, if M.A. units were earned in a private school.
6. One (1) copy thesis in book form as an additional requirement for a full-fledged M.A. graduate (with thesis).
7. For Credit Allowances Original copies of certificates of participation to any trainings attended with not less than 25 training hours.
Certificates of Participation which were already used for an earlier ERF may not be used anymore for the next application for upgrading. No credit allowances should be used more than once.
The applicant must be a School Head the person responsible for the administrative and instructional supervision of the school or cluster of schools. Must meet the Qualification Standards the minimum qualifications set in the new guidelines which include education, experience, training and civil service eligibility
Performance Rating should at least be VS for the last 3 consecutive years; or O for the last 2 consecutive years.
Teachers Support for HT I Six (6) teachers per school or cluster of 3 schools
List of teachers under supervision, with plantilla item number per latest PSIPOP - name of school or cluster of three (3) schools to be supervised (elem.) - list of teachers, their position titles and item numbers per PSIPOP.
For reclassification/upgrading of Teacher positions through the implementation of approved ERFs, the basic requirements are the following:

Teachers who have earned their diploma degrees from the UPOU will be considered on equal terms with those who have earned their masters degree from other institutions.
The conversion of existing teacher positions to Master Teacher positions shall be allowed provided that the lump-sum appropriations for the purpose shall not exceed the FY 2012 budget:
40.2.1 For elementary schools: Total Master Teacher Positions shall not exceed 10% of the total authorized teacher items in the district, to wit: a. Master Teacher I positions shall not exceed 6.6% of the total number of authorized teaching positions;
b. Master Teacher II positions shall not exceed 3.4% of the number of authorized Master Teacher I positions.
40.2.2 For secondary schools: One (1) Master Teacher position regardless of level may be allowed per subject area with at least 5-7 authorized teacher items.

Basic qualifications required: MASTER TEACHER I 1. Permanent teacher. 2. Bachelors Degree for teachers or equivalent as provided in Magna Carta for Teachers. 3. Very satisfactory performance rating for the last two years (at least 33 points). 4. At least three (3) years experience. 5. At least 25 points in leadership and potential or has been a demonstration teacher on the district level plus 15 points in leadership and potential.
MASTER TEACHER II
1. MT1 for at least one year. 2. Very satisfactory rating (at least 33 points) as MT1 3. Bachelors Degree for teachers or equivalent as provided in Magna Carta for Teachers, plus completion of academic requirements for M.A. 4. At least 30 points in leadership, potential and achievement or demonstration teacher on the division level plus 20 points in leadership and potential.

Funding for the Recurrent Cost of Salary Upgrading Approved in the past year, CY 2011:
- It shall be sourced from savings generated during the current year; - In case of inavailability of savings, it shall be charged from the lump sum allocation.
