FRA Fatigue Risk Assessment

Conduct a Fatigue Risk Assessment of the site with the Fatigue Management Coordinator utilizing the Risk Assessment in your Fatigue Management Guidelines. There are many risk factors associated with mining that need to be controlled in order to express the employers duty of care, The employer must use the principles of Risk management including the hierarchy of controls. (Insert company name here) will set up a special consultative group which will consist of one outside consultant, one Fatigue Management Coordinator and Fatigue Champions at each site to make their program sustainable.

Identifying Hazards
Are night shifts ever involved?
Are long hours of work required within a single shift? Are the work shift start times between midnight and 06:00? Have there been current changes to rosters? Is there repetitive or monotonous task involved? Are there complex physical or mental task involved? Do employees have excessive comminuting times? Are employees exposed to adverse working conditions?

Are non work related factors an issue? Sleeping disorders?

Assessing risks

Best Practices

The Head Office Fatigue Management Coordinator will review all new industry fatigue related best practices and bring to Stakeholders. The Fatigue Management Coordinator will consult with members from the Health and Safety committee to review all new proposed items before bringing on the mine. The Fatigue Management Coordinator along with the Health and Safety committee will review annually a matrix of technologies and compile a report of best practices and present to management.

Management Commitment

The Fatigue Management Coordinator will review Fatigue Management policies annually and establish top level management commitment. The policy will protect operational requirements and provide for companies vision, principles and said policies. Together they shall manage fatigue by reducing the risk of scheduled rosters, establish best principles for managing fatigue and implementing safety and fatigue management training to all employees.

Employee / Employer Partnership

The Fatigue Management Coordinator will meet with the Fatigue Champions at each site and establish buy in at the employee level. The unions shall be engaged as needed for employee support. The responsibilities of employee / employer are listed in the policy that follows and will acknowledge that total fatigue is equal to the employees roster & ergonomic and work factors & personal factors. In fit for duty it is the employees responsibility to show up for work in a rested state or let their supervisor know of their fatigue condition and for the employer to control the fatigue and overtime in the roster system.

Employee Education

The Fatigue Management Coordinator will educate the Site Fatigue Champions through a Train the Trainer Fatigue education workshop. The Fatigue Champions will educate the employees on Fatigue management annually. E-learning shall be established and reviewed. Fatigue Management Coordinator will attend outside workshops to keep fatigue education current with the industry. (Insert company name here) will strive to be a Global leader in Fatigue Management.

Conduct Staffing Analysis

Review all rosters and relief schedules using a bio-mathematical model to determine safest choice of rosters. Implement said roster with the buy in of all stakeholders including all unions and management.

Employee Responsibility
Employees (all personnel) must:

Make every reasonable effort to be fit for work whilst: At work On call Driving a company vehicle; or Operating company plant or equipment Notify their supervisor (host if visitor) if: Their work performance is likely to be affected by fatigue; or There is any risk to themselves or others due to the effects of fatigue; Notify their supervisor immediately (if a person will not self manage their potential impairment) where there is a potential risk to health and safety if they suspect others in the work place are behaving in a manner that suggests fatigue related impairment exists Encourage other personnel to comply with its procedure Participate in fatigue assessments, if required Rep-ort any actual or potential fatigue risks, and Fulfill their obligation that is, to arrive at work in a fit state All personnel are encouraged to use time away from work to obtain sufficient restorative sleep to ensure fatigue related risks are managed to an acceptable level.

Employer health sleep disorder screening

Health screening will be done by Occupational Health. All employees will undergo sleep disorder screening utilizing the EPWORTH TEST. All employees found to have a sleep disorder will be sent for additional testing. All employees will be made aware of services available to them in their Employee Assistance Program ie, counseling, drug and alcohol.

Control Mechanisms

Provide employee with safe schedules. Utilize collision avoidance systems on all moving vehicles Implement and install fatigue detection systems on mine, Supply shift workers with fatigue equipment ie earplugs, sleep mask to increase sleep quality. Fatigue education to all staff Energy drinks made available per shift.

Implement Fatigue controls

Upon orientation do Fatigue Management in e-learning. Provide Fatigue Management training annually to all employees. Do energy drink baseline twice a year to provide the mines with the safest energy drinks. Annual Fatigue Risk Assessment. Posters and one pagers on newest Fatigue Management ideas and equipment. Any new shifts or contractor schedules will be reviewed by Fatigue Management Coordinator. Fatigue Management Coordinator will also review Fatigue Management Plan and trailor to industry needs to create a safer mine.

Fatigue Management Coordinator along with outside consultant will audit the review process to keep it current and unbiased The audit will consist of: All current schedules Nutrition provided by mine Collision avoidance technologies Fatigue calculating systems Incidents and accidents and all reports that may contribute to fatigue.
