Dear ADENIJI FALILAT, OFFER OF PROVISIONAL ADMISSION INTO A 2-YEAR NATIONAL DIPLOMA PROGRAMME FOR THE 2011/2012 Session.
With reference to your application for admission into the Lagos State Polytechnic, I am pleased to inform you that you have been offered Provisional Admission for a 2-Year Full Time ND Programme for the 2011/2012 Session in Science Laboratory Technology in the School of Technology of the Polytechnic. Your admission is subject to the following terms and conditions, inter alia: (i) You are required to pay an ACCEPTANCE LEVY of N15, 000.00 with PORTAL ACTIVATION FEE of N3, 000.00 within 2 weeks of the date of this letter and submit duly completed ACCEPTANCE FORM. (ii) The Acceptance Levy is payable ONCE before commencement of Registration. Failure to pay promptly will lead to the forfeiture of this offer. (iii) You are to present the original of the relevant certificates (educational and birth) on which this admission was based and (8) eight passport photographs for verification. (iv) Verification and Registration by proxy are not allowed (v) As a Part-time student, you are not expected to be gainfully employed unless you are on duly authorized Study Leave. (vi) You should not be a registered student in another department, school or institution pursuing two programmes concurrently. (vii) This offer of admission can be nullified if it is discovered that it was made in error. (viii) This offer of admission will automatically lapse, become invalidated or be summarily nullified if at any point of your registration or studentship it is found that your entry qualifications are inadequate, fraudulent or questionable, or your claims are misleading and false. (ix) The Polytechnic reserves the inalienable right to prescribe fees payable by you during your studentship (x) Fees are payable on-line. Fees are NOT REFUNDABLE under any circumstance. (xi) It is mandatory for ALL STUDENTS to use their allotted Bursary Number for all payment transaction on the portal. (xii) Please NOTE that payment of all prescribed fees as stated in (x) is a pre-condition for registration and eligibility for examinations. (xiii) Whilst this provisional admission is for two academic sessions, you however have a maximum of eight semesters beyond which this offer lapses subject to the provisions of the Academic Policy of the Polytechnic. The conditions under which the admission can extend to 8 semesters are contained in the Student Handbook. (xiv) Late registration will attract a fee. (xv) Admission may be deferred for one session only, if a student duly qualified, has paid the prescribed fees and completed registration. CONGRATULATIONS.
