Directions: 1. To bus-stop X Board bus service 96/96A at Clementi Bus Interchange and alight at the 2nd stop after the bus passes across the flyover (over AYE). Other services to the bus-stop include 33, 183 and 188. 2. To bus-stop Y Meant specifically for student taking bus services 95 and 151. Alight at the bus-stop in front of Central Library / Arts faculty. If you are taking the Circle line, alight at Kent Ridge station and board internal NUS shuttle bus A1 or bus service 95. Alight at the bus stop in front of Central Library / Arts Faculty. Our camp facilitators will be stationed at the respective bus-stops (from 7.30 A.M. 8 A.M) to guide you to the building (SDE3). Upon reaching SDE3, take the lift to the 4 th floor and turn right after walking straight.

Students who are below 18 years old are required to have their parent/legal guardian complete this form. Students who are above 18 years old have to acknowledge by signing this indemnity form.
