Directions for completing the

OFFICIAL ENTRY FORM/CONTRACT


The enclosed Official Entry Form/Contract is the standard agreement for all contestants competing in the MISS USA & MISS TEEN USA state pageants. All contestants must complete this form/contract so that in the event that you are named your states delegate to the national pageant, no unforeseen questions arise regarding your eligibility to compete in the national competition. Simply follow the directions carefully, and return this entry form/contract with rest of the registration materials. If you have any questions, feel free to email or call us. DIRECTIONS

Please thoroughly read the complete Official Entry Form/Contract. ALL PAGES - Initial the bottom of every page of the Official Entry Form/Contract PAGE 1 - In the blank at the top of the page, fill in your name. PAGE 1 - In the blank at the top of the page, fill in the state in which you are competing. PAGE 1 - Attach photocopies of the residency documents as outlined below (and listed in Section 1, Item (e), Subsection 1 & 2). o I now reside and have resided in my State as my permanent and primary residence (as defined below) for a period of at least six consecutive months immediately prior to the commencement of the State Pageant (although I may have attended school, college or university on a full-time basis elsewhere) and, if employed, I am employed in my State (although I may occasionally travel outside my State on behalf of my employer). Permanent and primary residence means my true, fixed and permanent domicile which I intend to make my permanent home. In order to establish my compliance with these residency requirements, I am attaching photocopies of at least two of the following types of bona-fide documents, at least one of which must be of document type (a) through (e): (a) my drivers license; (b) my voter identification or registration card; (c) my resident tax return from the most recent tax year; (d) lease or deed for a dwelling in my name (evidencing occupancy of dwelling for at least six months immediately prior to the commencement of the State Pageant); (e) employment documents in my name (Form W-4 and pay stubs only); (f) my current school records (e.g., transcript or report cards with my address); (g) telephone or utility bills in my name (both the current bill and a bill dating back at least six months immediately prior to the commencement of the State Pageant); and (h) bank or credit card statements in my name (both the current statement and a statement dating back at least six months immediately prior to the commencement of the State Pageant). I will maintain such residency through the State Pageant and, if I win the State Pageant, through the 2012 Miss USA Pageant. I understand and agree that you have the right to reject information or documentation you feel has not been gathered or provided in good faith. I have resided in the State as a full time student at a school, college or university for one entire semester, trimester or summer session immediately prior to the commencement of the State Pageant and I am still enrolled as a full time student at my school, college or university and will continue to reside in the state (although I may have spent or be spending the summer in another State). In order to establish my compliance with these residency requirements, I am attaching bonafide copies of my current school records (e.g., transcript or report cards). I understand and agree that you have the right to reject information or documentation you feel has not been gathered or provided in good faith. I will maintain such residency through the State Pageant and, if I win the State Pageant, through the 2012 Miss USA Pageant.

PAGE 2 - Check TRUE of FALSE at the bottom of statement at Section 1, Item (e), and explain if the answer is FALSE. PAGE 2 - Fill in your birth date on the designated space on Section 1, Item (f). PAGE 2 - Attach a photocopy of your birth certificate, as stated in Section 1, Item (f). If you were not born in the United States, please attach a photocopy of your passport or naturalization papers, as stated in Section 1, Item (f). PAGE 3 - Check TRUE of FALSE at the bottom of statement at Section 1, Item (i), and fill in the information if the answer is FALSE. PAGE 3/4 - Check TRUE of FALSE at the bottom of statement at Section 1, Item (k), and fill in the information if the answer is FALSE. PAGE 4 - Check TRUE of FALSE at the bottom of statement at Section 1, Item (m), and fill in the information if the answer is FALSE. PAGE 8 - Check TRUE of FALSE at the bottom of statement at Section 9, Item (a), and explain if the answer is FALSE. PAGE 11 - Sign, date, and print your name on the designated spaces at the end of the document. If you are under the age of 18, have a parent/guardian sign, date, and print his/her name on the designated spaces at the end of the document. Return the Official Entry Form/Contract with the rest of the registration materials.
