Table Top Preparation

Intravenous Access 1. Two large bore catheters (18G, 20G) 2. 10 mL 0.9% Normal Saline Flush 3. Tourniquet, alcohol pads, gauze, plastic dressing, fenestrated tape, extensions 1. Syringe 20 mL: 1. Induction Agent: -Propofol (1-2.5 mg/kg)-only for patients that can tolerate decreases in MAP -Etomidate (0.1-0.4 mg/kg)-minimal impact on MAP -Thiopental (3-5 mg/kg)-cannot be used in asthma patients -Ketamine (1-2.5 mg/kg)-for patients that cannot tolerate decreases in MAP caused by general anesthesia 2. Syringe 10 mL: 1. Sympathomimetic: -Ephedrine (2.5-10 mg/kg) -HR & BP without organ hypoperfusion -Phenylephrine (1 mcg/kg) - BP without HR 3. Syringe 5 mL X 2: 1. NMBDs: -Atricurium (0.3-.5 mg/kg) -Nondepolarizing -Rocuronium (0.6-1.2 mg/kg) For Induction if SCh is contraindicated -Succinylcholine (0.5-1.5 mg/kg) -Depolarizing 4. Syringe 3 mL: 1. Antimuscarinics: -Atropine (0.01-0.02 mg/kg)- Vagalytic, treatment of bradycardia -Glycopyrrolate ( 0.005-0.01 mg/kg)- Antisiologogue 1. 2 vials: 1. Anticholinesterase: -Edrophonium (0.5 mg/kg)- Give with Atropine for Nondepol Block reversal -Neostigmine (0.08 mg/kg)- Give with Glyco for Nondepol Block reversal 2. 2 vials: 1. Opioid: -Morphine (1 mg/kg)- Induction, attenuation of ^ SNS 2/2 Laryngoscopy -Fentanyl (2-20 mcg/kg)- Induction, attenuation of ^ SNS 2/2 Laryngoscopy Undrawn medication 3. 1 vial: 1. Opioid Antagonist: -Nalaxone (4mg/1mL vial > dilute in 10 mL syringe with NS > give 1 mL (0.04 mg/5 minutes) 4. 1 vial: 1. Benzodiazepine -Midazolam (0.01-0.04mg/kg)- Sedation, amnesia, anxiolysis 5. 1 vial: 1. Benzodiazepine Antagonist: -Flumazenil (0.2mg/min) 6. 3 Ampules: 1. Local Anesthetic: -Lidocaine (2%) -Antiarrhythmic, Local Anesthetic

Intubation Equipment: -3 Endotracheal tubes (calculated size, size bigger, size smaller) -10 mL syringe for cuff inflation -2 Laryngoscope blades and 2 handles (ensure working light) -1 Guide wire -Water soluble lubricant -Tape 3.
