At the time of death Susan Smiths, sole surviving heirs were as follows: Rose Smith, 12 D St, San Rafael, California 94901, a Daughter, born February 06, 1991 I declare that, to the best of my knowledge and belief, the information herein is true, correct and complete as of February 24, 2012, the date I affixed my signature to this Affidavit. ______________________________________
