AFFIDAVIT OF LEGITIMATION PURSUANT TO RA 9858

(Legitimization of Children Born to Minor Parent/s)

That we, Jeffrie Reyes Galaban and Marielle Alexis Martinez Calip, both of legal age, Filipinos, husband and wife respectively and a resident of Sapang, Manaoag, Pangasinan, after having been duly sworn to in accordance with law do hereby depose and say: That we had lived together as husband and wife without the benefit of legal marriage as contemplated by the New Civil Code of the Philippines when out of our union as common law husband and wife without the benefit of legal marriage as contemplated by the New Civil Code of the Philippines we begot one child namely Ashley Vincent Calip Galaban born on 19 June 2009 at Manaoag, Pangasinan Philippines registered in the Local Civil Registrar of Manaoag, Pangasinan under civil registry number 09 962 on 24 August 2009. That the civil status of Ashley Vincent is an illegitimate natural child for the reason that when Ashley Vincent was conceived we were both single but were so disqualified to marry each other only because Marielle Alexis was below eighteen (18) years of age as contemplated by the New Civil Code of the Philippines. That subsequently on 24 October 2009 our marriage was solemnized by the Municipal Mayor of Manaoag, Pangasinan, Hon. Napoleon D. Sales, MD at his Office in Manaoag, Pangasinan and such marriage was simultaneously registered at the same town under registry number 09 274 on 29 October 2009. That we are subscribing to this affidavit for the purpose of informing the authorities concerned regarding the above facts and that this affidavit has been executed to change the civil status of our child from illegitimate to legitimate pursuant to Republic Act 9858 otherwise known as AN ACT PROVIDING FOR THE LEGITIMATION OF CHILDREN BORN TO PARENTS BELOW MARRYING AGE amending for the purpose Article 177 of THE FAMILY CODE OF THE PHILIPPINES, AS AMENDED.
