3. PHYSICAL RISK AUDIT (HAZOP) 1. Operators drive heavy machinery to load materials onto stockpiles.

The operators can be medically unfit to operate these heavy machineries. Due to sickness such as epilepsy, diabetes ect. Consequences may be that it will result in an accident where the employee can hurt himself and his co workers. 2. Operators drive heavy machinery to load materials onto stockpiles. Deviations may be that the operators are not trained. This can cause injury to people and damage to property. 3. The machinery intended to move these heavy materials can break. Machinery not being inspected can cause injury to people and damage to property due to faulty breaks ect. 4. The machinery is intended to offload materials at stockpiles the truck can tip over if it moves to close to the terrace or edge and can cause injury to people and damage to property. 5. The trucks offloading the materials at the stockpiles generate dust and can cause the operator of the vehicle to collide with other vehicles due to the fact that vision is obscured this can also cause damage to property. 6. The trucks moving the materials to the stockpiles can lose control over vehicles due to the wet roads and can cause damage to property and injury to people. 7. The operators traveling at high speed can lose control over the vehicles and collide with other vehicles. 8. Uneven roads between pick up and delivery points can cause materials to fall from the trucks onto other vehicles

COMMENTS: 7. PROPOSE RISK REDUCTION STRATEGIES 1. The operators of the vehicles should go for medical examinations and if they are declared fit they can operate the vehicles 2. Before appointing a new driver a thorough background check should be done by the person appointing them to determine if he is capable and qualified to do the work. As drivers licenses can be purchased it is of utmost importance that the background checks are done and appointments of drivers are done with great care. 3. Each driver should be assigned to his own vehicle which he alone will be driving and immediately inform the mechanic of faulty breaks or any other form of irregularities that may occur. The mechanic and the driver should be informed that no vehicles will be allowed to operate on the road if a problem exists. 4. A berm can be left at the stock piles to ensure that ADT operators do not move to close to the edges as well as the appointment of qualified people. 5. Watering the ground can reduce the risk of accidents due to dust. 6. Reducing the risk of accidents due to too much water the surface should be structured to encourage run off water. Warning signs can be placed on corners. 7. Road signs with appropriate speed can also reduce risks and drivers should adhere to all safety signs and obey the supervisors instructions at all times. 8. PROPOSALS FOR EFFICIENT AND EFFECTIVE RISK CONTROL: Operators of the vehicles should go for regular medical examinations as an illness can develop after a previous medical examination this will ensure that the risk will be controlled. A mechanic together with the driver should complete a checklist before vehicle is operated. Making time for maintenance of vehicles on a regular basis (downtime) to ensure that the vehicle is in working condition will also assist in controlling the risks. ADT operators must listen to excavator operators and tally men when they are warned signaled that they are to close to the edge or terrace The dust can be controlled through watering the ground Watercraft operators should be careful not to water the ground to much Operators must adhere to speed limits not faster than 40km/h The roads must be kept at level to prevent rocks from falling of the trucks. A safety car can be sent out before operations start to ensure that the roads are level.

9. CONCLUSION: It is important that operators of all heavy machinery be aware of the dangers involved and that they adhere to the rules set out. Most accidents occur due to human error and can have serious financial implications on the organization. Most accidents can be prevented if people take the necessary precautions.
