TO ALL EMPLOYEES NATIONAL TRAINING CENTER AND FORT IRWIN REQUEST FOR VOLUNTARY LEAVE DONATIONS 03/05/2012

1. Under the voluntary Leave Transfer Program, employees with emergency medical situations that are anticipated to result in a loss of pay for three work days may request voluntary leave donations. Due to the nature of the situation, the following employees fall under the provision of the Family Medical Leave Act. Any donations will help pay back any advanced sick leave as well as any leave without pay that may occur. The following employees needs your help:

1) Alicia Strickler. Account # FY 11, Ms. Alicia Strickler is requiring absence from work due to a medical emergency. 2) Allen Jackson. Account # FY 11, Mr. Allen Jackson is requiring absence from work due to a medical emergency. 3) Catherine Kazmierovicz. Account # FY 12, Ms. Catherine Kazmierovicz is requiring absence from work due to a medical emergency. 4) Jason Sinclair. Account # FY 12, Mr. Jason Sinclair is requiring absence from work due to a medical emergency.

2. Employees wishing to donate annual leave to the above account should submit their requests on OPM 630-A (OPM 630-B if outside agency) to the Directorate of Resource Management, Building 237, Room 135. 3. Employees with use or lose annual leave may wish to consider donation of leave as a means of exhausting excess leave. The maximum amounts of annual leave that may be donated during the leave year are as follows: Annual Leave Category 4hrs/pay period 6hrs/pay period 8hrs/pay period Maximum Donation 52 hours annual leave 80 hours annual leave 104 hours annual leave
