SCOPE AND LIMITATIONS In this study the researchers try to observe the student-nurses skills in nursing documentation correlating to the patients outcome. This study involve third-year student-nurses as the (correspondents) and patients as the (respondents) in the clinical area of any ward at University of the East Ramon Magsaysay Memorial Medical Center wherein there would be a permission letter provided for the nurse on duty in the one month observation lasting for five days per week, eight hours a day in every shift. The researchers guarantee that there would be neither treatment nor intervention given to the patient.
