Safety Rules for Inspection of Lifting Activities

Planning for the lifting operation: a) Arrange the contractor to use proper lifting appliances (LA) and lifting gear (LG), and establish safe system of works for lifting operation. For critical lifting activities, arrange the contractor to submit the method statement including lifting plan and the related risk assessment for approval. b) Arrange the contractor to submit relevant certificates of LA and LG, and the daily and weekly inspection records by competent person for site checking and record. Check the efficiency factor of the plant, in particular for the aged one, by comparing the safe working load assessed by the RPE shown on the statutory Form 3 with that on the load chart of the manufactures manual. c) Check that the colour coding system of Construction Site Safety Manual has been applied to LG. d) Whenever necessary, allow tolerance in estimating working radii due to deflection of jib under loading condition and wind effect on the suspended load. e) Arrange the contactor to assign competent coordinator for the high-risk lifting operation. Just Before the lifting operation: a) Check that the right LA and LG have been arranged on sound level ground prior to the lifting operation. b) Arrange the contractor to properly set up the LA and LG with full extension of outriggers, and ensure proper functioning of automatic safe load indicator and adequate security of the power-pack mounted on the back of the plant in both directions. c) When inspecting the LA & LG, wear cotton gloves wherever necessary for hand protection and be alert of the sharp edges. Arrange the contractor to provide the necessary manpower, plant and PPE for the inspection of heavy LG. Use plant instead of manpower to handle heavy items, whenever necessary. d) Check that the lifting plant operator has possessed the valid certificate. e) Coordinate with the contractors RSO or designated lifting supervisor to identify the lifting zone prior to lifting operation. f) Instruct the contractor to rectify the deficiencies promptly. During the lifting operation: a) Be alert of site condition and the weight involved. b) Wear safety helmet. Wherever appropriate, wear reflective coating for easy identification by the crane operator when working within or adjacent to the lifting zone. c) Whenever necessary, arrange the contractor to cordon off the lifting zone during lifting operation, and properly arrange, indicate and maintain the traffic route on site. d) Select a safe place to monitor the lifting operation. Avoid staying within the lifting zone, in particular not to stay in the area within the operating zone of jib and its back, during the lifting operation. e) Beware of the turning zone of an operating LA when approaching it. Keep a safe distance from it. f) Maintain effective communication with contractors lifting supervisor and RSO during the lifting operation. Rectify any identified deficiencies promptly on the spot. g) Be alert of the weather condition. Stop the lifting operation if the weather is getting adverse.

Safety Rules for Inspection to Virgin Land (including slope) Without Engagement of Contractor on Site
Planning for the inspection:a) b) c) Study the location map and identify safe routing. Provide introductory briefing to relevant officer regarding visit of virgin land. Whenever necessary, assign a competent person from the project team to conduct a specific risk assessment on the slope access to suit the individual situation. The competent person should have appropriate experience and relevant safety training on slope access. If considered necessary, the Project Team Leader may consider arrangement of appropriate safe access such as gangway or hydraulic mobile platform with the consent of the Architect via Term Contractor. For critical site inspection, assign a supporting officer staying in the station point with contact phone known to the project staff to effectively coordinate and provide necessary assistance to the project staff when conducting inspection on site. Names and contact phone no., inspection routing, and schedule of time required should be kept by the supporting officer (and also the individual project staff) for effective communication.

Before leaving for the inspection:a) Work in team of at least 2 personnel and carry a mobile phone. In case of remote site/island, work in team of at least 3 personnel so that in case of emergency at least one could immediately return to the effective communication point for emergency reporting to the Police and coordination with the supporting officer, and each should carry a mobile phone or walkie talkie where applicable for emergency preparedness. Wear proper light-coloured loose-fitting clothing with long sleeve and trousers, and suitable footwear. Bring along raincoat and the necessary personal protection equipment (PPE), such as hand gloves, safety helmet, etc., for use. Take sufficient drinking water. Whenever appropriate, bring along necessary equipment, such as map, compass, electric torch, etc., for emergency preparedness. Be alert of the weather condition and the weather broadcast. Review the situation and defer the inspection, where appropriate, if the weather condition is likely to become adverse (such as extremely hot weather, stormy weather, thunderstorm and lightning etc).

During the site inspection:a) b) c) Identify safe routing and ensure foothold for access/egress. Arrange indicator on spot to facilitate the retreat, where appropriate. Photos should be taken during inspection for future reference, wherever appropriate. Be alert of the site condition. Avoid staying in hazardous locations such as trench, watercourse, nullah, culvert, flooded-prone area and area in which the stability is in doubt, etc.

e) Apply insect repellent over the exposed parts of the body. If bitten by insect (e.g. mosquito, red imported fire ant) and the condition getting worse, retreat and consult doctor. f) Never put the hand into any unknown crevice, or touch or tamper with any snake. If a snake is encountered, keep calm and treat the snake with respect. Do not frighten or step over the snake. If bitten by the snake, identify the species of the snake, and report to the supporting officer for assistance. Apply appropriate first aid treatment, retreat and consult doctor. Wherever necessary, immediately report via 999 for help. Be aware of signs of heat stroke (symptoms such as mental confusion, loss of consciousness, hot & dry skin). Take a short break to recover under shelter whenever necessary. Be alert of the weather condition while staying on site. If the weather is becoming adverse with lightning, retreat to a shelter point as quickly as possible. Avoid staying in a position that is higher than the surrounding landscape. Seek a low spot and crouch in a position with feet together and keep away from tall or metal objects such as tree, telephone pole etc. In case of hill fire, keep calm and get away from the fire immediately. Try to retreat in the windward direction. Report the fire quickly to the Police by dialing 999, or dial at 2720 0777 to the Fire Control Center of Agriculture, Fisheries and Conservation Department. Report also to the supporting officer. In case of emergency: - Dial 999 or, whenever necessary, try international emergency call number 112. - Contact the supporting staff for assistance, wherever appropriate. The supporting officer should coordinate with the Architects Representative, Project Team Leader or the Architect for follow-up action including rescue. DSEAU should also be immediately informed via mobile no. 9093 2259 for appropriate arrangement of emergency reporting, and subsequently review the case. After the inspection:a) Report to the supporting officer by a representative of the project staff (normally decided by the most senior of the project team carrying out the inspection). In case of no reporting from the project staff by the end of the schedule, the supporting officer should try to contact with the project staff. Wherever necessary, the supporting officer should coordinate with the Architects Representative, Project Team Leader or the Architect for immediate follow-up. Should there be any accident, report as per Arch SD Operation Instruction no.13/03.

Safety Rules for Inspection of Demolition Works


Planning for the demolition works: a) Arrange the contractor to submit the appraisal report and the method statement together with the related risk assessment, and obtain the necessary approval prior to the commencement of any breaking works. Arrange the contractor to assign competent coordinators for identified high-risk activities on the spot. Identify the existing utility services, either above ground or underground, from utility record, and detect critically on the spot by the contractors competent person. Wherever necessary, arrange to properly divert, protect or abandon the utility services. Arrange the contractor to assign a registered electrical worker to install and maintain the temporary electrical system on site. Identify the location with asbestos material from the past record or PSB. Whenever necessary, arrange a specialist asbestos consultant to conduct an assessment and submit a report. Consult the client to recognize any biological hazards within the site and their related precautionary measures.

Before the commencement of the demolition works: a) Arrange the contractor to enclose the structure to be demolished with screen covers, and provide and maintain catch fan, debris chute and proper protective hood at entrance point, wherever necessary. Define the routing of the plant, and arrange the contractor to install adequate propping with bracings underneath. Wherever necessary, arrange the contractor to provide adequate fall protective measures including safety harness, dust screen and noise barrier on site. Check that the contractor has established a safe system of work for lifting operation, and proper lifting appliances and lifting gear have been arranged. Check that protruded objects have been covered up with cushions and indicated with warning by the contractor.

g) Check that the contractor has provided and maintained temporary drainage system and has established flood preventive measures on site. h) Arrange the contractor to adopt hydraulic crusher/saw cutter or the like to reduce the noise/dust nuisance caused by the demolition works. i) j) Arrange the contractor to conduct noise assessment to identify the ear protection zone and appropriate ear protectors for use on the spot. If entering into confined spaces is required, the site personnel should: - have attended at least the 1-day confined spaces course; and - arrange the contractor to conduct specific risk assessment for identified confined spaces and provide necessary measures to work within the confined spaces including the Permit-to-work system. k) If asbestos material is identified, arrange a specialist asbestos contractor to remove it first. During the inspection of the demolition works: a) Be alert of the site condition. Check the appraisal report and existing record, and identify the hazards on site. Strictly follow the precautionary measures, in particular on biological

hazards, recommended by the client, if any. Arrange the contractor to cordon off the affected area and clear the biological hazards prior to the demolition works. b) Arrange the contractor to carry out the demolition work in accordance with the approved method statement. Arrange the contractor to install proper tie and strut for the reinforced concrete column/ wall strip before cutting its base for taking down. Report any deviation to the Architect for immediate review and necessary rectification action by the contractor. Check that a safe access/egress, including the emergency escape routing, without obstruction has been maintained on site and provided with adequate lighting. Check that proper railing/barrier with toe board has been installed at floor edge and staircase landing, and opening not for debris disposal has been properly covered up with proper warning sign. Keep a safe distance from the opening. Instruct the contractor to cordon off the affected area during the course of breaking works, establish a permit-to-entry system wherever necessary, and install with local noise barrier or dust screen, wherever necessary. Control the accumulation height of demolition debris to prevent overloading of the structure. Ensure foothold during site inspection. Whenever necessary, arrange the contractor to provide proper gangway and suitable working platform for working-at-height activities. Arrange the contractor to maintain good housekeeping and remove grease and oil on site to avoid slippery floor. Wherever appropriate, install suitable curb at site entrance to prevent flow of surface water into the site. Arrange the contractor to spray water on spot to suppress dust during the course of breaking works. Arrange the contractor to clear up the affected area prior to hot-work cutting, and arrange local protective screen, where appropriate, to limit the sparks and strong light. Arrange the contractor to provide the necessary assistance for entering into the confined spaces and only staff having attended appropriate training on confined spaces will be allowed to enter the confined spaces. Wear proper personal protection equipment (PPE) including safety helmet with chinstraps wherever necessary, suitable safety footwear and, whenever necessary, appropriate facemask. When entering the ear protection zone, wear suitable ear protectors. Maintain the PPE properly. If removal of asbestos material is required on site: - Define restricted areas and arrange the contractor to establish a permit-to-entry system. - Implement preventive measures in accordance with the recommendations of the specialist asbestos consultant. - Wear appropriate PPE and follow the permit-to-entry system when entering the restricted area. Clean up immediately on site after the works. - Arrange the contractor to contain the asbestos material properly and dispose of by a licensed contractor. - Wherever necessary, arrange monitoring of air-borne asbestos fibre on site. o) Report to the Architect immediately if asbestos material is further encountered during the course of breaking works. Isolate the areas with asbestos material and arrange specialist asbestos contractor to remove it first.

Safety Rules for Visitor on Site


For effective arrangement of site visit for visitor, the project staff and the contractor should coordinate together to: a) b) c) d) Define a designated routing appropriate to the visitor. Establish an evacuation arrangement and a site emergency contact number. Provide adequate signage on prominent spots, including the warning signs and emergency contact number. Keep regular checking of the number of visitors during the site visit.

Before the site visit, the visitor should be arranged to: a) b) e) f) Wear appropriate clothing and footwear for the site visit. Register with site management/supervisory staff. Attend the safety briefing conducted by site management/supervisory personnel to understand the site safety rules and evacuation arrangement. Note down the site emergency contact number. Take care of the personal belonging.

During the site visit, the visitor should be arranged to: a) b) c) d) e) f) g) h) i) j) Be escorted by site management/supervisory personnel. Use the designated routing. Always wear the safety helmet (with chin strap if necessary). Follow the site safety rules and safety instruction given by the site management/ supervisory personnel. Put on appropriate personal protective equipment, such as mask, goggles, and ear protectors etc., as directed. Not smoke on site. Beware of the signage on spot, in particular the warning signs. Inform the site management/supervisory personnel if assistance is necessary. Follow the site management/supervisory personnel for retreat to the site office in case of inclement weather such as thunderstorm/lightning. Follow the instruction of site management/supervisory personnel and the evacuation arrangement if emergency situation is encountered during the site visit. Meet at the designated gathering point after the evacuation for further instruction from site management/supervisory personnel. Keep calm if loss of way is encountered during the site visit. Inform the site management/supervisory personnel by the site emergency contact number (check the site signage if the phone number could not be remembered). Stay along the designated routing and wait for the site management/supervisory personnel to come.
