I understand that as a Stylist I am not an employee, franchise holder or legal representative for bcharmed. I do not have any power of authority to incur any debt, obligation or liability on behalf of the company. I understand that I am working as a self-employed, independent contractor. As an independent contractor, I am responsible for ling all necessary federal, state and local taxes as may be applicable to any commissions, bonuses and awards I earn. In particular, I will not be treated as an employee with respect to any service for federal and state purposes. Also, I am not covered by any State Unemployment or Workers Compensation Act. I have read and agree to, the bcharmed Policies and Procedures. I will present bcharmed products in a truthful, sincere and honest manner, and I will conduct myself in a manner that reects the highest standards of integrity and responsibility, in keeping with the reputation of the company. I understand that bcharmed owns trademarks and copyrights for all of their materials, whether or not materials provided bear a trademark or copyright notice. I understand that information and materials provided to me contain CONFIDENTIAL AND PROPRIETARY INFORMATION about bcharmed. I authorize bcharmed to debit my credit/debit card to purchase product for the use of my business. I agree not to use, disclose or reproduce those materials for other than bcharmed business without the express consent of the company. This agreement is contingent upon acceptance by bcharmed Corporation Home Office. It is in effect for 12 months from the agreement date. Either party may terminate this agreement with 30 days written notice to the other. This agreement should be construed in accordance with the laws of the state of Utah. ARBITRATION OF DISPUTES - All disputes that may arise between the parties regarding the interpretation or application of this contract and the legal effect of this contract shall, to the exclusion of a court of law, be arbitrated and determined by a board of arbitrators, unless the parties can resolve the dispute by mutual agreement.
