Dear Doctors, Greetings. We congratulate Dr. A. Muruganathan for being Elected as National President Association of Physician of India, Dr. L.P. Thangavel for being Elected as President Elect TNSB - 2012 & Dr.C.N.Raja Our Branch Senior Member for being Elected as Vice President TNSB West Zone. Last Month We Conducted a Grand Cardiac Camp jointly with Shalimar Agencies, Chennai Apollo Hospital & Police Department. Arround 480 patients were screened 98 Patients Selected for Surgery at Apollo Hospitals. 13 Patients Surgery was Completed. Patients are doing well. We Thank Mr. Purushothaman for his Extreme Co-operation We Thank all Organizers especially Mr.Bharathi, Mr. Arasu & Mr. Balaji Chennai Apollo. The Academic Feast TIMA Research Day was well attended by all delegates. We Thank Dr.Easwaramoorthy for the Excellent work done by him. We also thank Dr. Sood from Malaysia who guided us to write a good Scientific paper and also we thank Dr. T. Sadagopan, Dr.J.A.Jayalal, Dr. Arjun Rajagopalan and Clini minds. Our West Zone fine arts and Sports meet was Conducted. Doctors from v a r i o u s branches participated and showed their talents. We thank Dr. Nancy & Dr. C.N. RECENT TRENDS IN DIABETES

CONTINUOUS GLUCOSE MONITORING -CGMS A continuous glucose monitoring system (CGMS) is an FDA-approved device that records blood sugar levels throughout the day and night. There are several approved devices -- Medtronic's MiniMed device, can provide up to 288 blood sugar measurements every 24 hours. The system is used to measure an average blood sugar for up to three days, while the person with diabetes continues daily activities at home. How Continuous Glucose Monitoring Works in Diabetes First, a tiny glucose-sensing device called a "sensor" is inserted just under the skin of your abdomen. The insertion is quick, and is usually not painful. It's very similar to insertion of an insulin pump catheter. Tape is used to hold it in place. The sensor measures the level of glucose in the tissue every 10 seconds and sends the information via a wire to a cell phone-sized device called a "monitor" that you attach to a belt or the waistline of your pants. The system automatically records an average glucose value every five minutes for up to 72 hours. INSULIN PUMP The Insulin Pump is probably the only device which can assure great control to Insulin dependant Diabetes patients, over and above other Insulin Infusion/Injection Devices. Not only do they provide the best control, they even help you lead a flexible and hassle free life, since Insulin Pumps provide pre loaded basal doses automatically. Apart from that, there is the freedom from multiple daily pricks. With Insulin Pumps, ideally you need around 10 pricks a month. In our country, where the diet is carbohydrate heavy, boluses can be taken accordingly, discreetly and at the touch of a button. In an insulin pump, insulin is transferred to the patient on a continuous basis. As big as a pen , a pump has a reservoir or cartridge for insulin, catheter, and a computer chip regulating the flow of insulin. The catheter, also known as infusion set or connecting device, is a thin plastic tube with a fine needle at the end. The pump can be attached to the waist belt just like a mobile phone. The catheter or the connecting tube of the pump is injected into the abdomen and filled with the short/rapid acting insulin, which is to be disposed off in three-four days. Insulin pumps are programmed electronically to deliver the desired amount of insulin and usually, fast acting analogues like lispro or aspart are used in the pumps

WRIST WATCH BLOOD GLUCOSE METER


Wrist watch blood glucose meter is also known as GlucoWatch. GlucoWatch is known to be the first FDA approved instrument which is not required to be inserted into the body for measuring the level of glucose. This device is noninvasive and totally automatic. It can measure the blood sugar level in every 20 minutes for nearly 12 hours. Principle of Wrist Watch Blood Glucose Meter The principle of working of this GlucoWatch is with the use of electric current to take out glucose painlessly and shift it into a transdermal pad which is medically known as Autosensor. This is connected to the backside of the back of the watch. The glucose level is estimated with the help of this auto sensor. This sensor uses electrons generated by a reaction caused by the electrical stimulus. It measures extracellular fluid that flows through the capillary walls . The glucose levels in this interstitial fluid is not directly equal to blood glucose, factors such as perspiration, oils, etc. can dilute the blood and adversely affect the results The sensors alone will cost $5,055 for one year's coverage (24 hours/day times 365 days/year divided by 13 hours/sensor equals 674 times $7.50 each). This means that the initial year's cost for the GlucoWatch itself and sensors will be $5,753. That's considerably more than test strips, which probably average about $0.75 each. So even if you test eight times a day, the strips would cost only $2,190 annually. Cygnus says that it doesn't know yet whether insurance plans will cover the GlucoWatch and its AutoSensors. Current available therapy: HUMAN ISLET TRANSPLANTATION endogenous sensor (beta cells) endogenous insulin secretion.

HOW OFTEN DOES THE PATIENT NEED HOSPITAL SERVICES


This is a quantitative exercise and may not be very difficult to handle. A hospital can monitor the frequency of a particular type of services, which are required by the patients more often on a seasonal, occasional and periodical basis and Endeavour to improve on a priority basis. This will not only make patients feel comfortable but would also reflect an image of competence. Patients and their attendants who visit hospital frequently usually develop interpersonal relations with staff and such interactions can generate considerable word of mouth publicity. The social skill of doctors and staff should be directed at winning the heart and mind through courteous and gentle behavior. In reality, the converse is true where staff and doctors feel that the patient has no other alternative. Such feelings are counterproductive because the moment an alternative means is found, patients are likely to move away from hospital. In case of chronic diseases, patients are compelled to visit the hospital frequently or at regular intervals. Hospitals should succeed in winning and retaining patients if motivated by a spirit of care and concern. This could secure tangible return in the long run. The use of statistical data would give an idea on the frequently of patients visit and hospital can predict on the basis of these data by comparing the data of previous years. This method would prove better as compared to guesswork or whimsical decisions, which tend to be risky. WHO DECIDES FINALLY WHICH HOSPITAL PATIENTS WILL GO? The process of decision making begins with the influence of the initiator, the decision-maker and then reaches the ultimate user. Factors interacting for the resulting outcome can be broadly summed up as: The characteristics of the services The characteristics of the provider and The characteristics of the situation Availability of broad base infrastructure and simplified procedure are preconditions before final decision can be reached. Factors such as: Location, outlook of the building, state-of-the-art equipment, communication network, parking, transportation, hospitality and attitude towards the patient contribute immensely towards final decision. Hospitals have realized the importance of guest relations officer, a concept that has made headway in retaining patients. Under the concept patients are no longer treated as customer and patients but as hospitals guest. This is possible where the staff are well acquainted with hospitals procedure and have developed a spirit of care, concern and commitment, Where such a concept is regularized in the system hospital have an edge over their competitors, the distinction of which can be felt by the patients, the attendants and the public at large. Hospitals adopting a culture of excellence need not feel threatened of growing competition. However, the crucial test for any hospital is that whether it is patient-oriented? In the new millennium, patients are increasingly being influenced by the characteristics of the healthcare providers. Therefore, the technical and social skill of doctors and the staff has to be on display in

order to make a profound impact in the minds of patient. Hospital owners and managers should involve patients and their attendants in the treatment plan, which would build better understanding and co-operation in the process of healing. Time is extremely valuable for everyone and therefore, OPD consultation, admission and discharge procedure should aim to minimize waiting time without ignoring delivery of core value. In handling such situation the hospital staff should be courteous in their dealing and maintain respect for the patient. Such gestures can rapidly build up public image and confidence in the eyes of its beholder. Hospitals should device a quality control in their set up which could position themselves better than their competitions. There are situations where patients are not conditioned by any of the above factors and it could be more emotional rather than rational. However such occasions are few. There are pressure from friend circle, family members and society at large to avail treatment from a particular hospital. Whereas the working class is subject to taking treatment from a particular hospital for the purpose of reimbursement. However, the ultimate choice depends upon the interaction of the patients with the characteristics of the services. The unique environment and the staff culture can bring about the final result. AT WHAT PRICE CAN PATIENTS AFFORD AND IS WILLING TO PAY ? Action speaks louder than words certainly applies to hospitals today who are under close scrutiny of the public. It also reflects the philosophy of the organization as to why it is there in the first place. Pricing ranges from profit oriented pricing, market share pricing, promotion pricing. The intention of healthcare provider can be easily understood by the public whether it is provide good services, mere venture, profit making or citadel of power. A hospital may have done its service planning, physical distribution and promotion policy well; but if it fails to adopt a sound pricing strategy it can put the hospital out of joint. Instances of such hospital abound. Pricing is the hub where the wheel of services planning, distribution and promotional policy actually revolves. Hospitals would be wise to take into account the following inputs before fixing its price: The general awareness of the community which could enhance or reduce their willingness to pay for healthcare services The income level of the community The provision of value added services The potential demand of the community for that particular services The target group: Mothers, Under Fives, Retired, Accident-prone The cost involved in promotional activity Allocation for charity Time involved in paying back loans Reaching a break-even point on a fixed/given period Predetermined rate of return expected from the project It is not easy to understand why patients have developed certain attitudes over a period of time towards a hospital. It is equally difficult to understand why and how a patient is holding certain

opinion as more important and relevant than the other. Hospital managers should be able to grasp patients felt need when faced with conflicting choice. Observing patients reactions over a period of time could provide an in-depth knowledge in their behavioral aspect. Managers can then formulate an effective strategy for keeping them happy today and tomorrow. Patients and their attendants often face the following questions: How much to spend today and how much to save for tomorrow? How much money should be spent on hospital bills and in what way i.e., health insurance, monthly budget.What options are available during contingency?What interest rate will be applicable in case credit is availed? Within the patients environment there are other pressing needs involving financial commitments Patients will then have to think of alternative means for financing when faced with multiple need situations. Since patient wants to satisfy a variety of needs ranging from physical, safety, sense of belonging, status and self-actualization. At several occasion s/he is not aware of the needs which motivates him for action. The final choice is determined by complex factor interacting within the patients personal environment. To begin with is the fulfillment of basic human needs, the class of users, the type of services expected, the status symbol of the promoter/owner, the timing of services, its system and procedures and the mode of payment play a very crucial part in determining the final choice.

Inter Branch Fine Arts Competitions for west Zone@ ERODE on 9th Oct
October 9th Erode IMA Hall was packed with IMA doctors and their families from West zone. It was a pleasant surprise to see our collegues from all corners of west zone . We had all the events registered with sportive involvements and colourful performances. Morning session, we had the children's events and it was carried-on upto lunch, with so many participants. Dr.Ramadevi, Dr. Gomathy, Dr.SreeDevi, Dr.Jansi from Udumalaipet with their wards came in the morning itself , took part in Sports events and participated in the Fine Arts events with the same enthusiasm. Dr. Ramadevi deserves a special appreciation for bringing their enthusiastic team ; also she played a very good instrumental number with Veena. Dr.Jansi& Dr.Sreedevi played key-board; Dr.Gomathy has won the solo singing with her nice voice. Dr.Raveendranath was a versatile singer. He represented Tiruppur branch as one-man army and participated in all events and won many prizes for their branch. He sang, Danced, played Instrumental number and also took part in the sports events in the morning. Dr.P.M. Saraswathi from Gobi branch who has crossed 60 years came with an enthusiastic spirit and won the first prize for solo singing without instruments. No words to appreciate her involvement . Credit goes to Dr.Chandrika who personally came earlier to rehearse her song and motivated Dr.Saraswathi to participate. Dr.Josephine Mary from Sangagiri brought her son to perform an wonderful selfcomposed song with key board which was much appreciated by the audience. Like-wise Dr. Rajeswari from Palani brought her sons to sing, dance & play key- board. Her son is a very good dancer. Above all the team from Coimbatore lead by Dr.Sekar Michael was the real threat to the home-team. Their instrumental orchestration was really nice & the voices of his daughters were simply superb and made the competitors sweat a lot. Master Pranesh was a mastero in key-board. The home-team from Erode was with dedicated practice. This time they went one step ahead with superb dance performances. Keerthana, Monika, Krithika, Alice, Sashmitha ,Adhi All are Doctors' children danced with solo, group, classic, free-style and made us proud of their performance. Dr.Radhakrishnan, Dr.Eswaramoorthy, Dr.Ezhi, Dr.Selvan, Dr.Suseendhar, Dr.Prince,Dr.Chitra, Dr.Nirmala, Dr.Anitha, Dr.Saraswathi & their Master Dr.Santhi mesmerized the crowd with their Group dances . Dr.Chitra Soundarrajan was the talk of the day with her solo dance . Everyone was appreciating the dedicated work of our Doctors and made us happy by their praises and blessings. Our State President ,Dr.Sadagopan, who is the man behind all these successful stories was with us through-out the day and encouraged our participants. 4 AAO GAON CHALEN At Swaminathapuram 4320 Patients were treated in the month of October 2011. Our IMA Members t o o k part in it.Screening Camp was Conducted at SKM Sevai Maiyam, Saminathapuram. SATHYA SAI Sri Sathya Sai Seva Samithi Erode Conducted General Camp. Total Number 152 Patients. Our members participated in it. ANAEMIA FREE INDIA Cheran Hospital 72 Patients Treated. ERODE GH 125 Free Dialysis were done at Govt. Head Quarters Hospital, Erode. 45 Free Endoscopic examination were done. BLOOD DONATION CAMP In October 2011, 07 Blood Donation Camps were held. Total of 700 Units of Blood were Collected. ARASAN EYE HOSPITAL 14 Free eye camps , 1224 patients screened, 231 Free surgeries performed , 60 pairs of eyes collected. KMCH SPECIALITY HOSPITAL KMCH Erode conducted free Osteoporosis & Bone Density Detection Camp on 20.10.2011. Total number of patients screened - 110. SENTHIL MULTI SPECIALITY HOSPITAL A free Neuro Medical Camp was Conducted by Senthil Multi Speciality Hospital, Erode on 16.10.2011, Sunday at Milan Hospital approximately 80 Patients screened. MONIKA DIABETES CENTER Oct 1 Free Diabetes Detection a Awareness Programme was conducted. Totally 200 patients were screened. LOTUS HOSPITAL Free Medical Check up 120 patients benifited. ADI SATHYA HOSPITAL One Hb A1c Free camps, Five free Spirometry and Asthma detection camps. MAARUTHI MEDICAL CENTRE AND HOSPITALS Erode Diabetes Foundation functioning in Maaruthi Medical Centre and Hospitals, conducted free Diabetes Detection and Awareness Programme on 30.10.2011, on Diabetes and Prevention of foot Complication. Free Blood glucoseestimation done for around 210 patients and for other potential diabetics. ASOGEM CAMP ASOGEM General Medical camp conducted 150 Patients Benifited. . 4

Dear colleges, Greetings from IMAYAM. Our noble project IMAYAM has been serving our society beyond our expectation. Since 5 years, so far 550 terminally ill cancer patients lived in IMAYAM under our tender care, until their last breath. IMAYAM completes five years on 18.12.2011. We have been running smoothly without any interruption. However, we have to raise finance to run in future. Hence we request you to kindly sponsor 1 day's expense during your birthday, son's/daughter's birthday or wedding day by paying just Rs.1500/- for one day's expense and have the satisfaction of taking part in this human service.
