Side Effect/Adverse Reaction Binds to Hypotension dopamine Constipation receptors in Sweating the CNS. Also Nausea and has Vomiting anticholinergi Hyperglycemia c and alphaExtrapyramidal adrenergic reactions blocking Sedation activity. Dizziness Produces Dry Mouth fewer Salivation extrapyramind Weight Gain al reactions Fever and less Abdominal tardive Discomfort dyskinesia than standard antipsychotics but carries high risk of hematologic abnormalities.

Action

Nursing Consideration -Monitor the blood pressure and pulse rate before administering the medication -Monitor for signs of myocarditis (unexplained fatigue, dyspnea, tachypnea, fever, chest pain, palpitations). If this occur clozapine should be discontinued and not restarted -Monitor for any possible side effects of tardive dyskinesia.Rep ort this signs immediately, may be irreversible -Monitor for frequency and consistency of bowel movements. Increasing bulk and fluids in the diet may help to minimize constipation -Overdose of this medication is treated with activated charcoal and supportive therapy.

Patient Teachings -Instruct patient to take medication exactly as directed -Inform patient of possibility of extrapyramidal symptoms. Instruct patient to report the symptoms immediately -Advise patient to change positions slowly to minimize orthostatic hypotension -Instruct patient to use frequent mouth rinses, good oral hygiene, and sugarless gum or candy to minimize dry mouth -Instruct patient to notify health care provider promptly if unexplained fatigue, dyspnea, tachypnea, chest pain, palpitations, sore throat, lethargy, weakness and malaise

Nursing Consideration -Monitor for frequency and consistency of bowel movements.
