In exchange for good and valuable consideration given or promised to be given by the

Producer to me, I acknowledge that I have been informed of the nature and purpose of the abovementioned student production (the Production) and my involvement in it, and I hereby give my irrevocable and unconditional consent to the Producer for the recording of my voice, likeness, characterisation, silhouette, performance and poses (the Recording) for the purposes of the Production, and for the exploitation or other use of the Recording, without limitation, in any and all media by any manner or means whether or not now known or invented, used or contemplated, throughout the world for the full period or periods of copyright protection, including all renewals, reversions and extensions, created or provided by the laws of any country.

I acknowledge that for the purposes of the Production, it may be necessary for the

Producer to edit, erase, duplicate, add to, digitise or adapt the Recording, in whatever manner it sees fit.

3.

I acknowledge that the Producer is the sole owner of the copyright in the Production.

4.

I warrant that I am not prevented, contractually or otherwise, from granting my

permission in respect of the making of the Recording, and I agree to indemnify the Producer against all claims, liabilities, damages or expenses arising from the breach of any of my undertakings under this agreement.

5.

I agree that I will not maintain or bring against the Producer, its assigns, licensees or

successors any demand, suit, claim or action of whatever nature, involving but not limited to actions based on breach of confidence, defamation or infringement of any other civil right arising out of the authorised use of my voice and physical likeness in the Production as provided above. The Producer, its assigns, licensees and successors are hereby released from all legal and equitable liabilities, demands, claims, expenses and actions of whatever nature, whether known or unknown, anticipated or otherwise, that may now or in the future arise in connection with the Producers authorised use as described in this document.

This document is to be governed by and construed in accordance with the laws of

Singapore, the courts of which shall be the courts of competent jurisdiction.
