TUSKA RIDGE HOMEOWNERS ASSOCIATION ARCHITECTURAL REVIEW COMMITTEE REQUEST FORM INSTRUCTIONS PLEASE READ THESE INSTRUCTIONS CAREFULLY

1. The Architectural Review Committee (ARC) request form must be completed and approved BEFORE any work commences on the property. 2. The approval process can take up to thirty (30) days from the date the ARC receives your request; therefore, please plan accordingly. 3. The property owner shall complete the entire form and sign it. The following information MUST be included (as applicable): details and specifications regarding any building, fence and/or landscaping change(s), installation(s) or addition(s) including dimensions, plans, estimates or pictures as necessary; brand name(s), color(s) and number(s) of any paint that will be applied to house or fence; fence material, style, etc.; any other additional information you feel may be important. 4. For house painting, you MUST paint a square of the desired (main) color (~ 3 x 3 feet), as well as several linear feet of the desired trim color, so they are visible from the street (color chips will NOT be accepted). This should be done before the original request form is submitted. For fences, if the color is either white or the same as the house main or trim color, just make note as such; otherwise, a 3x3 test patch will also be required on the fence. Once the ACR inspects the paint color (at least two ARC members must approve or deny a paint color), the ARC request form will be completed and a copy returned to you. 5. Even if you plan to repaint your house or fence in the same color, you MUST submit for approval, since the original (prior) color may have been inadvertently approved, it may not have been approved or may have been discontinued by the manufacturer. 6. For ALL requests except those involving only house or fence painting (i.e., building, fence and/or landscaping changes, installations or additions), a copy of the property survey or plot plan must be submitted with the locations of such, drawn to scale. 7. The one-page ARC request forms without any additional information attached (or only black and white 8 x 11 inch plans, estimates, etc.) may be faxed to the following number to expedite processing: 407-774-8375 8. ARC request forms that include information in color, pictures, photographs, large plans, etc. must be delivered or mailed to the Tuska Ridge Homeowners Association property management company (Sutherland Management) at the following address: Tuska Ridge HOA c/o Sutherland Management Inc. 107 N. Line Drive Apopka , Florida 32703 7. Sutherland Management will process and forward your request to the ARC. The ARC may solicit input from the Board of Directors (which meets once per month) in making their decision. ALL decisions are based on the fair and impartial application of the rules and regulations specified in the Declaration of Covenants, Conditions and Restrictions of Tuska Ridge. Any denials of requests may be appealed to the Tuska Ridge HOA Board. 8. Please DO NOT call Sutherland Management they do NOT have any decision-making power in this matter and have NO information on the status of your request. 9. Once the request is approved or denied, the ARC will submit a copy of the original request to you (which may include additional comments from the ARC explaining any additional conditions of approval or the reason(s) for disapproval, as appropriate) and return the original request form to Sutherland Management. 10. All contact regarding the ARC approval process should go through the Tuska Ridge property management company (Sutherland Management). Please do not contact members of the board or ARC members.
