Daily Action Plan

DATE: January 9, 2011 Week/Day: 2nd week/3rd day

Leader of the Day: Ms. Marchelle Lea M. Dulay Section/Group: BSN114/Group54

I. AREA OF CONCERN

This day the group will be focusing on Dry run of Mano po, Bisyo ay iwasan, kalusugan ay
pangalagaan, “Kalusugan ay pahalagahan, High blood and Diabetes ay iwasan, and Iwas Wild disease
program. Also the group will be conduct a follow-up home visit to families, and will be targeting on
having seed planting and finalizing the spot map.

II.OBJECTIVES
After 8 hours of community organizing this day, the group plans to accomplish the following objectives:
1. Dry run of Mano po and Bisyo ay iwasan, kalusugan ay pangalagaan, “Kalusugan ay pahalagahan,
High blood and Diabetes ay iwasan, Iwas Wild disease program
2. Seed planting
3. To conduct a follow-up home visit to families.
4. To finalized the spot map of the barangay.

III. ACTIVITIES
To achieve the following objectives, the following activities will be accomplished:

8:00-8:30 Pre-conference at the Pavillion

8:30-10:00 Follow-up home visit to families
10:00-11:00 Making announcement and giving of invitation in the community regarding the
program.
11:00-12:00 Lunch Break
12:00-12:40 Seed planting
12:40- 3:00 Dry run of Mano po and Bisyo ay iwasan, kalusugan ay pangalagaan,
“Kalusugan ay pahalagahan, High blood and Diabetes ay iwasan, Iwas Wild
disease program
3:00-3:40 Finalized the spot map of the barangay
3:40-4:00 Post-conference and evaluation
4:00 Dismissal
IV. TARGET
To make the objectives possible the group should be able to have a successful dry run of the said
programs and be able to come up with a complete and accurate spot map. And also to established
rapport and have a continued assessment of the families through home visit.

V. RESOURCES

Besides the aforementioned target, the following resources will be needed by the group to accomplish the
objectives of the day:

MANPOWER: BSN114/ Group 54

MONEY: The group needs money for the printing of the project proposal with the amount of
Php 2 per page.
MATERIALS: seeds, props and costumes, Community Health Nursing Book are needed to achieve the
objectives mentioned.

After 8 hours of community organizing the group was able to:
