WORK EXPERIENCE:
Springfield Hospital Center, Social Worker 1, HS: November 2007-present: Duties
include evaluating and assessing severely and persistently mentally ill patient
s diagnosed with schizophrenia and DSM-IV diagnosis to develop and maintain trea
tment and discharge planning. Participating in interdisciplinary treatment team
meetings on a daily basis, working with Psychiatrists, Occupational Therapists,
Registered Nurses, Direct Care Aides and other Social Workers. I participate i
n treatment activities and case management; providing psycho education to patien

ntally ill at Key Point Health Services, Inc. Duties include: Administration/su
pervision of Direct Care staff (Mental Health Counselors), assist Program Manage
r in day to day operations of the program and serve as acting Program Manager in
the absence of the manager. Assessing/interviewing clients to identify appropr
iate referrals to residential rehabilitation programs. Facilitating group, indiv
idual and family therapy to determine the type of living arrangement that best m
eet client's needs, process all forms necessary for placement, clarify client's
strengths and needs for the rehabilitation plan, develop policies and procedures
for case management functions as needed, meet all time lines and documentation
as set forth in COMAR regulations governing Residential Rehabilitation Programs,
coordinating/updating all treatment plans to be forwarded to MHP.
