State was a combined program of cardiac and thoracic surgery, supervised by Pro
fessor Johannes Meyer. During the training at the University of Cape Town, six
months were dedicated to thoracic surgery and the remaining time to cardiovascul
ar surgery. This was directly supervised by Professor Christiaan N. Barnard.
In 1981, I was appointed specialist in cardiac surgery at Groote Schuur Hospital
and Red Cross War Memorial Children's Hospital in Cape Town, South Africa. Thi
s appointment was held until January 1987. During this period, I was actively i
nvolved in adult and pediatric cardiac surgery, with main emphasis in the field
of heart transplantation. Part of my duties included the training of residents,
participation in ward rounds, tutorials, and supervising a group of researchers
studying of the effects of brain-death on the heart and its effects on the meta
bolism of the body as a whole.
From February 4, 1987, until October 1990, I was a member of the heart transplan
tation team at the Oklahoma Transplantation Institute, Baptist Medical Center, O
klahoma City, Oklahoma.
Since November 1990, I have been working as an Associate Professor at the Univer
sity of South Florida in the Department of Cardiothoracic Surgery in Tampa, Flor
ida.
As of July 1998, I have been promoted and received tenure as a full Professor at
the University of South Florida in the Department of Cardiothoracic Surgery in
Tampa, Florida.

SCHOLARSHIPS
During my medical training, I was initially a junior tutor of anatomy, and after
graduation during the rotating internship, I became a lecturer of anatomy with
the Anatomy Department, La Plata Medical School, Buenos Aires, Argentina, 1965-1
970.

CLINICAL WORK

During the years of training in general surgery, I was exposed to all surgical s
pecialties on a rotation basis during the first two years.
As Chief Resident, I was in charge of the Department of General Surgery, being r
esponsible for all the emergency operations and permanently on call for trauma c
alls. During this year, I performed all -types of major general surgical proced
ures.
As Resident in Cardiothoracic Surgery, I was guided by Professor C.N. Barnard in
the field of cardiovascular surgery. My primary responsibilities were initiall
y in the care of patients and, during the last two years, performing all types o
f adult cardiac operations and pediatric cardiac surgery.
From the beginning, my duty was especially dedicated to the care of donors for h
eart transplantation. After one year, I took over the post-operative management
of patients who had undergone a heart transplant.

I performed my first heterotopic heart transplant as a fellow at Groote Schuur H


ospital.
In September 1982, I was appointed by Professor C.N. Barnard as a specialist in
charge of the Heart Transplant Unit and full-time Fellow at the Cardiac Research
Laboratory for one year. During that period, I developed a non-invasive techni
que for the diagnosis of acute cardiac rejection using radionuclide-scanning met
hods, first in animals and then afterwards applied the technique to human cardia
c transplantation, which was widely used at the University of Cape Town.
From January 1984 to the present, I have continued my research activities. At t
he Heart Transplant Unit in Cape Town, I worked approximately 50% in the laborat
ory where I embarked on the experimental study of brain-death in baboons and pig
s looking at the effects on the myocardium and the hemodynamic and endocrine det
erioration following brain-death. The metabolic studies in brain-dead animals a
nd patients confirmed that aerobic metabolism is inhibited. Explaining one of t
he mechanisms participating in the graft dysfunction after organ transplantation
.
Based on this work, I developed the "hormonal therapy for brain-dead organ donor
s", showing its beneficial effect.
As a result of this research, I found that by administering Triiodothyronine (T3
), the metabolic dysfunction is fully corrected prior to organ transplantation.
Currently, this therapeutic modality is used widely in the rescue of unstable b
rain-dead organ donors.
Together with Dr. W.N. Wicomb, I was involved in myocardial preservation in a po
rtable unit, achieving successful preservation of the myocardium over a period o
f 48 hours, followed by orthotopic transplantation of the hearts of Chacma baboo
ns with excellent survival of the animals.
In immunosuppressed Chacma baboons, I performed using cardiopulmonary bypass, an
en bloc heart and lung transplantation finding that the lungs are actively reje
cted while minimal histological changes occur in the transplanted hearts. This
is in contraindication to previous reports in which it was stated that the heart
and lungs are rejected simultaneously. This work was published in the Annals o
f Thoracic Surgery.
In September 1982, I was in charge of the Department of Heart Transplantation, w
orking in close collaboration with Professor Christiaan N. Barnard, and later wi
th B. Reichart and D.K.C. Cooper, performing personally a total of 55 heart tran
splants, of which 8 were retransplantations (1 for irreversible acute rejection
and 7 were for chronic rejection). In 2 patients, the recipient heart was resec
ted (previous
heteroptoic heart transplants), and a heart was implanted in orthotopic position
, leaving these patients with two transplanted hearts. During this period, 12 h
eart and lung transplants were performed in Groote Schuur Hospital, in which I p
articipated actively in the surgical procedures, resecting the donor heart and l
ung block and performing a heart and lung transplant in a patient with primary p
ulmonary hypertension.

In 1987, as a member of the Oklahoma Transplantation Institute, I became co-dire


ctor of research and director or organ procurement. While in this position, I h
ave been involved in 77 procedures, including heart and heart-lung transplants.
From 1990 to December 1995, I held the title of Acting Chief of Cardiothoracic S
urgery at the James A. Haley Veterans' Hospital. On January 1, 1996, I became t
he formal Chief of Cardiothoracic Surgery performing the majority of open heart
surgeries. I am also involved in Thoracic Surgery coverage whenever required an
d provide surgical stand by for all PTCA elective or emergent.
At Tampa General Hospital, Tampa, Florida, I am a member of the Heart Transplant
Team. I perform open heart surgery and provide coverage for the Thoracic Traum
a Service.
From July to present, I have performed over 1,000 total myocardial revasculariza
tions not using the heart-lung machine. This is a new re-introduction of old te
chnology. However, with the advent of beta-blockers and special retractors, I a
m able to do a coronary bypass on any epicardial artery. This technique has bee
n performed only in one other institution in the United States.

PREVIOUS RESEARCH

The potential applications of Triiodothyronine therapy following acute myocardia


l ischemic events were further examined in conditions exhibiting the euthyroid s
ick syndrome.
Experimental work was performed at the Veterinary School at the Oklahoma State U
niversity in Stillwater and the Oklahoma Medical Research Foundation. The main
emphasis of this research was to examine the effects of Triiodothyronine in the
reversal of myocardial dysfunction in the following conditions: 1) stunned Myoc
ardium, 2) recovery of the cardiac function following catecholamine infusion, an
d 3) myocardial depression following cardiopulmonary bypass.
Currently, we are looking at the effect of T3 pretreatment in rats with heart fa
ilure, observing a significant cardiac recovery and response to beta agonists.
Control rats, after six weeks of heart failure, no longer respond to beta agonis
ts. This may become of relevance mainly in open heart surgery or following PTCA
's.
I have been acting as a Consult to SmithKline Beecham Pharmaceuticals for the de
velopment of protocols related to T3 in experimental and clinical applications i
n non-thyroid diseases.
The FDA has issued an IND number for the use of T3 in non-thyroid diseases. A r
andomized blinded study was done in Oklahoma in patients undergoing coronary art
ery grafts on cardiopulmonary bypass.
Recently, a fully funded study has been completed (Protocol TS-107). In this st
udy using transesophageal echocardiography, we examined the effects of T3 in pat
ients undergoing open heart surgery in a randomized, prospective blinded study.
This was done in collaboration with Dr. Hector Fontanet (Cardiology Department)
.
We administered open T3 to high-risk heart patients with an estimated mortality
of 15-50%, (using a mathematical model developed by Dr. Parsonnet from Beth Isra
el Hospital, New Jersey), observing a significant death rate reduction by 60%.
At the James A. Haley Veterans' Hospital, I was the Primary Investigator for a V
A Cooperative Study entitled, "Urgent Revascularization in Unstable Angina".
In addition, I was Co-Investigator in the following research protocols:

tilation and Perfusion in Patients After Cardiac Operations?
5) Can Dobutamine Stress Echocardiography Predict Recovery of
Contractile Function of Viable Myocardium?
I was a Primary Investigator at the Baptist Medical Center for the use of the "T
horatec" ventricular assist device for the rescue of the myocardium following op
en heart surgery and as a bridge to transplantation. I implanted the device in a
patient who survived 70 days with its support (one of the longest ventricular a
ssist device supported patients in the USA), and was thereafter successfully tra
nsplanted.

CURRENT CLINICAL RESEARCH

I am Co-Chairman with Dr. Fred Grover on a VA grant (10.6 million dollars) for a
cooperative study aOutcomes Following Myocardial Revascularization: On and Off
Cardiopulmonary Bypassa, which was performed in 18 Veteran Medical Centers.
We have completed the randomization and one year follow up for the Cooperative s
tudy CSP 517. Preparation of the main manuscript on the study findings has been
initiated.
I am the senior author of the primary manuscript of the study CSP 517 entitled:
Outcomes Following Non-Emergent Coronary Artery Bypass Graft Procedures:
Revascularization Off versus On Bypass (ROOBY) Trial Findings. NEJM
Multiple manuscripts will follow the primary paper, I estimate that at least twe
nty relevant papers will be published in prestigious journals and multiple meet
ing presentations will follow.
Also, I am reviewer for selected articles for the following journals: 1) Annals
of Thoracic Surgery, 2) Transplantation, and 3) The Journal of heart and Lung Tr
ansplantation.
