REASON FOR EVALUATION:
Lori was born a twin and they are adopted. Her biological father beat her severely and as a result from one of
the beatings, she sustained a fractured skull. A bone fragment lodged into her brain and consequently lesions
has formed. Her twin does not display any academic problems.

CASE HISTORY:
1. Developmental History
Lori, by all accounts, is a ‘normal’ child. She does not exhibit any outward signs of the physical
abuse. Every other aspect of her development is normal, except the processing area of the brain.

2. PHYSICAL DEVELOPMENT
a. General Health/Physical/Motor
Lori overall health and physical appearance is of a normal child.
b. Medically relevant Conditions
As a result of the abuse, Lori has epilepsy. She has permeant lesions on her brain from the skull
fragments.
c. Prenatal and peri-natal information

2. LANGUAGE DEVELOPMENT – Describe how this child communicates
a. Phonetics Does the child pronounce words clearly and correctly? Give
Examples of any pronunciation errors.
Lori’s pronunciation is below her grade level. She does not pronounce words correctly, and it from her reading
issues. She is far behind the other students.

b. Does the child use proper grammar? Give examples of any grammatical errors cited in the book.
Lori does not use proper grammar, this is caused by her not being able to read or write, and process written
symbols. Examples of Lori’s improper grammar and sentence structure would be when Lori was talking with Tom
and she asked “Littler than me?” Another example is when she was talking Boo and ask “What you supposed to leave
alone, Boo, leave what alone?” She does not use proper verb relations in her sentence structure.
c. Semantics – quote several of this child’s “bigger” vocabulary words. This means active/productive
vocabulary). How does his/her vocabulary compare with that of other children the same age?
Lori’s vocabulary is not at grade level. She maintains the vocabulary of a much younger child. Her exposure to
new vocabulary is directly related to her inability to read and write. She is easily frustrated when reading and reading
exercises are presented to her. The result of her frustration is that she shuts down and is not receptive to learning.
d. Receptive – Does this child understand instructions given by his/her teachers?
Lori has difficulties in receiving written instructions. She does not process the written symbols, therefore she
cannot read or write. This results in her not completing school work and low grades. She is constantly being punished
