Mrs. Scammel was admitted to your unit one week ago with a diagnosis of
discharged tomorrow. When you go in her room to make your rounds, Mrs.
Scammel tells you that her vision is blurred and she has pain in her right eye. The
conjunctiva of the eye is red and looks inflamed. You also notice some crusty
drainage on her upper and lower eyelids. You suspect the client may have an
infection in her eye or she may have injured her eye. You want to notify the MD

SBAR ACTIVITY #1ANSWER
Mrs. Scammel was admitted to your unit one week ago with a diagnosis of
discharged tomorrow. When you go in her room to make your rounds, Mrs.
Scammel tells you that her vision is blurred and she has pain in her right eye. The
conjunctiva of the eye is red and looks inflamed. You also notice some crusty
drainage on her upper and lower eyelids. You suspect the client may have an
infection in her eye or she may have injured her eye. You want to notify the MD
S-My name is_____. I am calling regarding your patient Laura Scammel Rm 206. She is c/o
blurred vision and pain in her right eye. She is scheduled to be discharged tomorrow.
B-She was admitted to the hospital one week ago for a UTI. She has dementia and is on the
following medications.
A-The conjuctiva of her right eye is red an inflamed and there is some crusty drainage on her
upper and lower eyelids. If able to do a vision test on her give the results-her near and far vision
are blurred. There is no report of any injury by patient or staff.
R-I suspect that Mrs. Scammel may have conjunctivitis or she may have injured her cornea.
Could you please come and see Mrs. Scammel before she is discharged tomorrow. What should
I do to relieve her symptoms until you come?
SBAR ACTIVITY #2
Mr. Kane is a 45-yr-old male admitted to the hospital one week ago for Bronchitis
and exacerbation of his Asthma. He was transferred to your medical unit 2 days
ago from the ICU. Mr. Kane has Type 2 diabetes and is receiving Albuterol via
nebulizer Q4H and is on Prednisone 5mg po QID. Mr. Kane has an order for
blood glucose monitoring ac and at bedtime. His fingerstick before dinner is 305.

ANSWER

WHAT TO DO FIRST?
Before calling MD check previous BSL results, check chart for sliding scale order and check
MAR for other meds and any other pertinent information. Keep information with you when you
make call to look up other info MD may request from chart.
S-Name, Unit, I am calling about your patient Mr. Kane in room 520. His BSL before dinner is
305.
B-Admitted one week ago for bronchitis and asthma exacerbation. He was intubated and in ICU
for 4 days. He was transferred to this unit 2 days ago. He has Type 2 diabetes, on nebulizer
treatments and is receiving. Prednisone 5mg QID; Glucophage 500mg BID and Duricef 500mg
QID. BSL range has been 140 -200 since his hospitalization.
A-BSL 305 before dinner. V/S stable and pulse oximetry is 99%. Offers no complaints at this
time.
R-I am concerned about his BSL and he is due for dinner. Could you give me a V/O for regular
insulin to cover his meal-there is no sliding scale coverage. When do you want me to monitor his
BSL again? Should I call you with the results even it is normal?
