TEACHING PLAN FOR URINARY TRACT INFECTION

DESCRIPTION OF THE LEARNER: A 59 year old female, with proper body physique, is now suffering from Rectal CA stage IV and with a history of type 2
DM. Nutritional assessment revealed that the client eat foods high in cholesterol and carbohydrates; she also usually prefers to take in fast foods, like in Jollibee.
She stated also that her water intake is very minimal like she takes in half of a glass every meal; she prefers to take in salty foods and junk foods. Physical
assessment revealed her current urinary problem; she stated that she has painful urination. She stated also that she has urinary tract infection before and usually takes
in 2-3 buko juice a day to treat her condition but not now. The client appears neat and in proper clothing. The client is able to understand, speak and communicate
using Filipino and English as a means of communication. Furthermore, the client has no visual or hearing impairment.

LEARNING NEED: How to prevent urinary tract infection.

LEARNING DIAGNOSIS: Knowledge deficit on the prevention of urinary tract infection related to lack of knowledge as manifested by the reoccurrence of the
disease process.

GOAL: The client is able to understand and apply preventive measures for UTI.
