This new collection of WHO information products reflects the work and priorities of the
Organization and provides guidelines and standards for medical, technical and scientific
professionals working in many different fields.

When the first edition of this manual was published in 1995, it quickly
established itself as a reference work. In the intervening period, the
rapid developments that have occurred in both ultrasound equipment
and investigative techniques, including use of ultrasonography in
the therapeutic domain, have necessitated publication of a totally
new edition of this manual.

The present manual, a basic reference text that covers ultrasound


techniques, recognition of normal anatomical features and
differential diagnosis, is the first of two volumes of the second
edition. Volume 2, which will cover paediatric examinations,
gynaecology and musculoskeletal examinations, as well as
therapy, will appear at a later date.

These new publications, which extensively cover modern diagnostic


and therapeutic ultrasonography, will be of great use to medical

professionals in both developed and developing countries.

Volume 1 begins with a chapter on the basic physics of ultrasound,


including one dimensional A-, B-, and M-mode, B- mode two-
dimensional, three-, four-dimensional and Doppler ultrasound.

ultrasonography of each of the main organs of the body. The

To improve survival and quality of life among the 2.5 million


children living with HIV, a comprehensive package of
prevention, care and treatment is required. This package
should include management of infections such as pneumonia,
diarrhoea, malaria and ear infections, as well as common
opportunistic infections and HIV-related co-morbidities.
WHO is developing a series of guidelines on each of these
conditions, following the GRADE approach.

The document on the management of pneumonia and


diarrhoea in HIV-infected infants and children is the first of
this series. The recommendations are similar to those for non
infected children, but they cover specific aspects related to
HIV infection.

This publication is intended primarily for use by national


advisory boards, national HIV/AIDS and child health programme
managers and senior policy-makers involved in planning
national strategies for child survival and paediatric prevention

on Disability

Following the Convention on the Rights of Persons with Disabilities,


attention has focused on ways to remove barriers to the participation
of people with disabilities in their societies. Now a new publication,
the first ever World report on disability, produced jointly by WHO
and the World Bank, reviews evidence about the situation of disabled
people, and offers directions for policy and practice, which can
help overcome exclusion.

The report contains chapters on health; rehabilitation; assistance


and support; enabling environments; education; and employment.
The voices of people with disabilities themselves open each chapter,
setting the scene for what follows. Within the chapters, case studies
show how many countries have succeeded in promoting good practice.
The diverse challenges faced in low, middle and high income settings
are differentiated wherever possible.

Importantly, the report also includes the first new disability
prevalence estimates since the 1970s. Global trends such as
the ageing population, and the increasing importance of
chronic diseases, mean that the prevalence of disability
is likely to rise in the future. The report makes the case for
better data collection, using the International Classification
of Functioning, Disability and Health. Overall, there are
major evidence gaps in the disability field, and the report
argues that more research and disaggregated data are
needed to show what works to improve the lives of
persons with disabilities.

This pioneering report shows how removing barriers to


mainstream services and investing in rehabilitation, support
services and more accessible environments can ensure that millions
of people with disabilities can participate in education, employment

When working with countries to measure and compare health systems


functioning, it is important to strike a good balance between avoiding
blueprints that do not allow for country contexts and specificities
while also encouraging a degree of standardization that enables
comparisons within and between countries as well as over time.
Standardized indicators allow comparisons between countries
and can help mutual learning, including the identification of
bottlenecks and the sharing of lessons learnt.

This handbook does not attempt to cover all components of


the health system or deal with the various monitoring and evaluation
frameworks. Instead, it is structured around the WHO framework that
describes health systems in terms of six core components or "building
blocks": service delivery, health workforce, health information
systems, medical products, vaccines and technologies, financing
and leadership/governance. The selection of indicators was guided 15
by the need to detect change and show progress in health systems
strengthening. Indicators relate to both the level and distribution of
inputs and outputs. While the focus is on low- and middle-income
countries, experiences from high-income countries are also used
to guide the development of measurement systems.

Each section has proposed core indicators that all countries are
encouraged to collect, plus a wider set of indicators that users
can choose or modify as needed. It is anticipated that the core
indicators will enable the production of country "dashboards"
that contain the instruments by which health systems trends
can be regularly monitored and compared. Countries should
integrate new indicators with existing indicators of their health
sector and statistical strategies and plans. Health systems
monitoring should also be seen in the context of the indicators'
impact on access to priority health services and their contribution
to reaching the Millennium Development Goals (MDGs).

covers one health system component or building block and
is set out along the following lines:

• description of possible sources of information and available
measurement strategies;
• proposed "core indicators", supplemented, where necessary,

WHO recently launched the mental health Gap Action Programme


(mhGAP) for low- and middle-income countries with the objective of
scaling up care for mental, neurological and substance-use disorders.
This mhGAP Intervention Guide has been developed to facilitate
mhGAP-related delivery of evidence-based interventions in non-
specialized health-care settings.

The present model guide is based on a review of all the science available
in this area and presents the interventions recommended for use in low-
and middle-income countries. The Intervention Guide includes guidance
on evidence-based interventions to identify and manage a number of
priority conditions. The priority conditions included are depression,
psychosis, bipolar disorders, epilepsy, developmental and behaviour
disorders in children and adolescents, dementia, alcohol-use disorders,
drug-use disorders, self-harm/suicide and other significant emotional
or medically unexplained complaints. These priority conditions were

selected because they represent a large burden in terms of mortality,
morbidity or disability, have high economic costs, and are associated
with violations of human rights. The guide is aimed at health-care
providers working at first- and second-level facilities.

The Intervention Guide is brief so as to facilitate interventions by busy


non-specialists in low- and middle-income countries. It describes in
detail what to do but does not go into descriptions of how to do.

The Blue Trunk


Library
The Blue Trunk Library has been developed by the library of
the World Health Organization for installation in district health
centres in Africa as a means of compensating for the lack of
up-to-date medical and health information. After 12 years
of existence more than 1900 libraries have been distributed
to health districts in poor countries of the
world, where health information access is
difficult. Training workshops, for the optimal
use of the information contained in each
blue trunk, was also provided to more than
550 health professionals during this period.

The collection, which is organized according


to major health subjects, contains more
than one hundred books on medicine and public health. The
collection has recently been revised and updated with the latest
information available.

Among the works chosen, priority has been given to practical

manuals (especially those published by WHO) offering easily
accessible solutions to the medical, public health and
management problems medical staff may have to face.

Differing levels of education among district medical staff


have also been taken into account. The same topic may
be addressed in different publications from a different point
of view: that of the physician, the nurse, the nursing
auxiliary or health worker.

Les cinq dernières années ont été l’occasion pour les acteurs de la
RBC de travailler en étroite collaboration pour la mise en œuvre De

et leurs familles ’ soient en mesure d’accéder aux prestations des
secteurs de la santé, de l’éducation, des moyens de subsistance et des

(‘empowerment’) en facilitant l’intégration et la participation des
personnes handicapées, de leurs familles et communautés dans tous
les processus de développement et de prise de décision.

“L’établissement d’un réseau de diffusion de l’information
médicale et sanitaire au delà des circuits universitaires et
académiques a été rendu possible par le projet “Bibliothèques
bleues”. La cible des bibliothèques bleues, ce sont les centres
de santé du district. Les livres sélectionnés ont été choisis
pour cette cible.”

La Bibliothèque bleue est une collection qui


a été mise au point par la bibliothèque de
l’Organisation mondiale de la Santé pour être
installée dans les centres de santé de district en
Afrique afin de pallier le manque d’information
médicale et sanitaire actualisée.

Cette collection qui est pré-classée par grandes rubriques,


contient une centaine de livres portant sur la médecine
et la santé publique.

Pour faciliter son transport et sa conservation, la collection

d’ouvrages a été emballée dans une malle en tôle bleue qui
a été équipée de deux étagères sur lesquelles sont classées
les boîtes à brochures numérotées contenant les livres.

Parmi les ouvrages sélectionnés, priorité a été donnée aux


manuels pratiques, (en particulier ceux publiés par l’OMS),
dans lesquels on trouve facilement la réponse aux problèmes
de santé et de gestion sanitaire auxquels le personnel médical
peut être confronté. Il a également été tenu compte des
différents niveaux de formation parmi le personnel médical
du district, c’est pourquoi le même sujet peut être traité
dans plusieurs ouvrages mais sous un angle à chaque fois
différent : celui du médecin, de l’infirmier(e), de l’aide-
soignant(e) ou de l’auxiliaire de santé.

las brechas en salud mental

La Guía de Intervención incluye recomendaciones sobre las


intervenciones basadas en datos probatorios para identificar
y manejar una serie de condiciones prioritarias. Estas condiciones
prioritarias incluyen: depresión, psicosis, trastorno bipolar, epilepsia,
trastornos del desarrollo y conductuales en niños y adolescentes,
demencia, trastornos por uso de alcohol, trastornos por uso de drogas,
lesiones autoinfligidas/suicidio y otros síntomas emocionales significativos
o no justificados médicamente.

La Guía de Intervención es breve para facilitar las intervenciones


por parte de los profesionales no especializados, sobrecargados
laboralmente, en los países de ingresos bajos y medio-bajos. La Guía
incluye un "esquema principal" que proporciona información sobre
las formas más comunes de presentación de las condiciones prioritarias
con el fin de guiar al clínico a los módulos pertinentes. Los módulos

suministran información sobre como evaluar y manejar las condiciones
prioritarias con un formato simple de árbol de decisiones.

La Guía de Intervención es una herramienta técnica creada por la OMS


para ayudar a la implementación en los países del Programa de Acción
Mundial para superar las brechas en salud mental (mhGAP).
