En el caso más frecuente, que es la sospecha de atonía uterina, los pasos que
se deben seguir una vez establecido el diagnóstico son los siguientes, entendiendo
que no son en todos los casos sucesivos, y que en ocasiones deberán ser
simultáneos, quedando todos ellos condicionados por la gravedad y etiología del
cuadro clínico y teniendo en cuenta que siempre deben quedar correctamente
reflejados en la historia clínica:

- Embolización selectiva radiológica: puede ser una alternativa útil en los centros
donde se cuenta con radiólogos intervensionista y cuando la hemorragia no
resulta una amenaza urgente para la vida. Nivel de evidencia/recomendación III
B.
Este procedimiento sólo debe ser ofrecido una vez revisado el cérvix y
vagina, asegurada la evacuación uterina y cuando tras el uso de uterotónicos
persiste el sangrado. Sus indicaciones incluyen la atonía uterina, la hemorragia
cérvico-uterina, las laceraciones cérvico-vaginales que no se resuelven
quirúrgicamente y la placenta acreta. Esta técnica consigue resolver la
hemorragia en el 94% de los casos.
