KNOW ALL MEN BY THIS PRESENT:

This contract, made this _____________,by and between Mr./Ms./Mrs.______________,

Of legal age, single/married, with postal address at ______________________________
(hereinafter) referred to as the EMPLOYER.

Mr./Ms./Mrs. _______________,born on___________,single /married/widow, with postal

address at ____________________________.Hereinafter called the EMPLOYEE.

CHERKIE’S EMPLOYMENT SERVICES, with residence and postal address at Lot 26 Blk 13
Phase II Paraluman St.Suburbia East Subdivision, Parang Marikina City, hereinafter refered to as
Cherkie’s, hereby agrees as follows;

1.That the EMPLOYER hires _____________as _____________ for the period of

____months commencing from the date hereof;

2. That the EMPLOYER shall pay ____________a monthly salary amounting to

P_________, payable every 15th and 30th of the month.

3. That the EMPLOYER shall pay CHERKIE’S an AGENCY Fee covering for a period
of six (6) months contract amounting to P4,000 in which the amount is NOT REFUNDABLE
NOR TRANSFERRABLE and SHALL NOT BE CHARGED against the EMPLOYEE.

4. That DURING THE DURATION OF THIS Contract, the EMPLOYER is entitled to

two (2) replacement subject to AVAILABILITY of an Applicant,provided that the EMPLOYER
SHALL PAY A REPLACEMENT FEE amounting to P________, provided further, that the
EMPLOYER, in order to get a replacement shall return the EMPLOYEE to
CHERKIE’S, in accordance with Condition No.7 of this Contract EMPLOYMENT PERIOD is
good for six (6) months from original contract date and/or for two (2) replacements, whichever
comes first.

 5.That the EMPLOYEE hereby authorize to pay in advance to CHERKIE’s the sum of
P___________ corresponding to _____%(percent) to the EMPLOYEES SALARIES charged on
the first month only,and reimbursed the amount of P_______,
representing the cash advance by the EMPLOYEE from CHERKIE’S.

6. That the EMPLOYER SHALL NOT MOVE OR TRANSFER, the employment of

said EMPLOYEE to any other person without prior written permission to CHERKIE’S ,
otherwise, the EMPLOYER shall shoulder all responsibilities resulting thereto.

7. That the EMPLOYER, shall return to CHERKIE’S the EMPLOYEE, anytime he/she
would leave the service for proper accounting of wages and other benefits that may due him/her.

8. That the EMPLOYER shall provide EMPLOYEE with stay-in agreement with
ADEQUATE FOOD and LODGING, in addition, the EPMPLOYER shall allow the
EMPLOYEE, to at least 1 (ONE) day off every month. Provide further that if the EMPLOYEE
opts not to avail his/her day off, but instead perform his/her regular duties during said period, the
EMPLOYEE shall be ENTITLED to ADDITIONAL PAY in proportion to his/her monthly
salary.

9. That the expiration or termination of this contract. The EMPLOYER-EMPLOYEE

relationship may be continued on MUTUAL AGREEMENT.

10. That the EMPLOYER Is deemed to have forfeited his/her right for a replacement
without cost of refund of the service fee, if he/she FAILED to avail of the same WITHIN ONE
(1) MONTH from the date of engagement of the EMPLOYEE.

11. That the EMPLOYER recognizes the right and authority of CHERKIE’S
to withdraw the services of the EMPLOYEE incase of any violation of this contract or any
law,rules and regulations promulgated by DEPARTMENT OF LABOR AND EMPLOYMENT
(DOLE).

12. That the EMPLOYEE assumes exclusive responsibilities FOR HIS/HER

PERSONAL ACTS OR CRIMINAL otherwise, and that the EMPLOYER SHALL NOT HOLD
CHERKIE’S liable therefore.

13. That the provision to Chapter II, Section I,Title VIII, of the Civil Code particularly
Articles 1690,1691,1694,1695,1697 and 1699 shall constitute part of this Contract.

14. CHERKIE’S WILL NOT BE RESPONSIBLE for personal traits,loans and salary
advances.
