REQUISITE DOCUMENTS FOR FAMILY REUNION VISA

MANDATORY DOCUMENTS FOR CHILDREN

1 Application form fully completed and signed by the applicant
Two recent passport-size photographs (white background). Please do not staple the
2 photographs to the application form.
Passport having validity of 3 months from date of expiry of visa with 1 blank page, issued
3 within the jurisdiction of the Embassy/Consulate *
“Nulla Osta” (No Objection Certificate) in original, issued by competent Questura in Italy.
Kindly note that the “Nulla Osta” is valid only for a period up to six months from date of issue.The “Nulla
4 Osta” will have to be renewed on expiry.
Discrepancy, if any, in the names of the applicant/s on the “Nulla Osta” and the
5 passport/s, should be corrected to correspond with the name on the passport.
Attested photocopy of the passport (all pages) of the person who has applied for the
6 “Nulla Osta” in Italy. If available, also of the old passport.
7 Attested photocopy of the “Permesso di soggiorno” (Permit of stay).
Ration card of the family, translated in English language. Both, original and translation
must be duly attested by the local Authorities and verified by the Ministry of
8 External Affairs,New Delhi.
Verification of original documents and of the English translation of Birth Certificate,
Marriage Certificates, Ration Cards & other relevant documentation.
* If any change has occurred in this regard an adequate proof of residence needs to be submitted
9 (Bank account, telephone/electricity bill etc.)
Birth Certificate of the child/children in original, issued by the Registrar of Births
and Deaths, clearly mentioning the registration number, along with translation in English
language, both verified and authenticated by the local Authorities and verified at the
Ministry of the External Affairs, New Delhi. In case birth is registered at a later date, as per
Indian custom, the Birth Certificate is to be made on the order of the SDM (Sub Divisional
Magistrate). Copy of the order is to be submitted along with Birth Certificate. If the
10 applicant’s date of birth precedes 01.04.1971
An “Affidavit” from the parent in India giving his/her consent to the child traveling
to Italy for family reunion with the parent residing in Italy. The name of the person
accompanying the minor should be mentioned with a copy of that person’s passport and air ticket. In case
the minor is traveling unaccompanied, the name of the airline should be
11 mentioned.
If both parents are resident in Italy, then a “dichiarazione” equivalent to the
“Affidavit” has to be done and attested by the competent Authority in Italy. In the latter
12 case a copy of identity document of the parent /s in Italy has to be enclosed.
13 Photocopy of all observations in the passport

PLEASE NOTE: All documents must be submitted in original. Documents such as Birth &Marriage
certificates, School Leaving Certificates must be submitted in original and photocopy. Photocopies to be
done ONLY after attestation by the local Authorities (SDM/Dv. Commissioner and Home Department) and
verification at the Ministry of External Affairs, New Delhi. The above mentioned originals (Birth & Marriage
Certificates and School Leaving Certificates) will be RETURNED after the visa processing
