Formula for Determination of Hemodynamics in Cardiac Catheterization

(By Fick Formula) (Written by K.C. Yen, CGU)

1. Cardiac output (Qs)(mL/min) = systemic flow =

O2 intake (mL/min)
X100
O2 content of arterial blood (mL/dL) – O2 content of mixed venous blood (mL/dL)

2. Pulmonary artery flow (Qp)(mL/min) =

O2 intake (mL/min)
X100
O2 content of pulmonary venous blood (mL/dL) – O2 content of pulmonary arterial blood (mL/dL)

3. Effective pulmonary artery flow (Qep)(mL/min) =

O2 intake (mL/min)
X100
Pulmonary venous O2 content (mL/dL) – mixed venous O2 content (mL/dL){SVC or RA}

4. Total Left  Right shunt = Qp - Qep

5. Total Right  Left shunt = Qs - Qep
6. Pulmonary vascular resistance (PVR) = (mean PA – mean LA)(mmHg) / Qp (ml/min)
7. Systemic vascular resistance (SVR) = (mean Ao – mean RA)(mmHg) / Qs (ml/min)
Other Formula
O2 content (mL O2 / dL blood + plasma) =
1.36(mL O2 / gm Hb) × Hgb(gm Hb / dL blood) × SaO2(%) +
{ 0.0031[(mL O2 / dL plasma) / mmHg] × PaO2(mmHg) }  we can neglect the number of this portion
in formula 1, 2, 3 (because it was too small)

Body Surface Area (BSA) (m2) = W0.425 × H0.725 × 0.007184 (Du Bois Method)
= (W × H / 3600)1/2 (Mosteller method)
(weight in the unit of kilogram, height in the unit of centimeter)

O2 intake = Oxygen consumption [(mL O2/min ) /m2] × BSA (m2)

(Oxygen consumption should check the Table in textbook)
In this case, we give the O2 saturation of different location in heart to measure hemodynamics of
patient. To simplify the statistics, we design the saturation of mPA=LPA=RPA is 85%, SVC=RA is
75%, RV=PV=LA=LV=Ao is 95%. The Hgb is 13.9 gm%, body weight is 9.06 (kg), body height
is 70 (cm), Oxygen consumption is 170[(mL O2/min ) /m2]

BSA = 9.060.425 × 70 0.725 × 0.007184 = 0.399 (m2)

170 [(mL O2/min ) /m2] × 0.399 (m2) × 100

Qs =
1.36(mL O2 / gm Hb) × 13.9(gm Hb / dL blood) × 95% + α(mL O2 / dL plasma) - 1.36(mL O2 / gm Hb) × 13.9(gm Hb / dL blood)
× 75% + β(mL O2 / dL plasma)

67.83 (mL O2 / min) × 100

= = 17.94 × 100 (dL blood + plasma / min)
3.7808 (mL O2 / dL blood) + (α-β) (mL O2 / dL plasma)

= 17940 (mL blood + plasma / min) [(α-β) is neglect]

170 [(mL O2/min ) /m2] × 0.399 (m2) × 100

Qp =
1.36(mL O2 / gm Hb) × 13.9(gm Hb / dL blood) × 95% + κ(mL O2 / dL plasma) - 1.36(mL O2 / gm Hb) × 13.9(gm Hb / dL blood)
× 85% + λ(mL O2 / dL plasma)

67.83 (mL O2 / min) × 100

= = 35.88 × 100 (dL blood + plasma / min)
1.8904 (mL O2 / dL blood) + (κ-λ) (mL O2 / dL plasma)

= 35880 (mL blood + plasma / min) [(κ-λ) is neglect]

170 [(mL O2/min ) /m2] × 0.399 (m2) × 100
Qep =
1.36(mL O2 / gm Hb) × 13.9(gm Hb / dL blood) × 95% + κ(mL O2 / dL plasma) - 1.36(mL O2 / gm Hb) × 13.9(gm Hb / dL blood)
× 75% + β(mL O2 / dL plasma)

67.83 (mL O2 / min) × 100

= = 17.94 × 100 (dL blood + plasma / min)
3.7808 (mL O2 / dL blood) + (κ-β) (mL O2 / dL plasma)

= 17940 (mL blood + plasma / min) [(κ-β) is neglect]

Total Left  Right shunt = Qp – Qep = 35880 (mL blood + plasma / min) – 17940 (mL blood + plasma / min)
= 17940 (mL blood + plasma / min)

Qp 3588 (mL blood + plasma / min)

mL blood + plasma
Qs 1794 ( / min)

The mean pulmonary artery pressure is 22 mmHg, mean artery pressure in LA is 9 mmHg, mean
artery pressure in Ao is 64 mmHg, mean artery pressure in RA is 7 mmHg.
PVR = (22 - 9) (mmHg)/Qp = 13(mmHg)/35880(ml/min) = 3.62 × 10-4 (mmHg.min / ml)
SVR = (64 - 7) (mmHg)/Qs = 57(mmHg)/17940(ml/min) = 3.1 × 10-3 (mmHg.min / ml)
