Line Responsibility: The organisational philosophy, whereby the
responsibility for an activity passes "down the line"

Sub-supplier: A generic term which is used to refer to any

3.0 RESPONSIBILITIES

3.1 BELMONT ASIA -NDT & QUALITY SERVICES Senior


Management

BELMONT ASIA -NDT & QUALITY SERVICES CEO has the


ultimate responsibility to define BELMONT ASIA -NDT &
QUALITY SERVICES HSE Policy and to ensure the application of
this manual to all BELMONT ASIA -NDT & QUALITY
SERVICES activities.

The responsibility for the application of BELMONT ASIA -NDT &


QUALITY SERVICES HSE policy and this manual to the specific
logistic job has been delegated to BELMONT ASIA -NDT &
QUALITY SERVICES Representative.

3.2 BELMONT ASIA -NDT & QUALITY SERVICES


Representative

BELMONT ASIA -NDT & QUALITY SERVICES Representative


is responsible for the Health and Safety of all BELMONT ASIA -
NDT & QUALITY SERVICES and sub-supplier and/or Joint
venture site employees and for compliance with the environmental
protection measures established for the job.

The Representative's primary HSE responsibilities are as follows:


 To ensure the full and effective implementation of BELMONT
ASIA -NDT & QUALITY SERVICES HSE policies;

 To take a direct interest in HSE matters and to ensure that


adequate resources are available to fulfil HSE requirements;
 To ensure compliance with CLIENT Contract and Local
Pakistan Regulations and the requirements detailed in this
procedure;
 To confirm that accidents and incidents are being properly
reported and investigated;
 To ensure the availability of senior personnel to participate in
site safety inspections and accident investigations as required.

3.3 BELMONT ASIA -NDT & QUALITY SERVICES and Sub-


supplier Personnel

BELMONT ASIA -NDT & QUALITY SERVICES HSE


Management System is based on the understanding that BELMONT
ASIA -NDT & QUALITY SERVICES and Sub-supplier employees
are individually and collectively responsible for their own and their
colleagues’ safety and for the observance of environmental
regulations and requirements.

All employees have the following safety obligations:

 Attend safety induction meetings and toolbox meetings.


 Read and comply with BELMONT ASIA -NDT & QUALITY
SERVICES HSE policy.
 Use all appropriate safety equipment.
 Operate all equipment and tools, including vehicles, strictly for
the intended purpose.
 Comply with all instructions concerning the use of harmful
materials.
 Not to remove, interfere with or otherwise modify machinery
guards and other safety equipment.
 Immediately report any accident or incident to their supervisor.

3.4 BELMONT ASIA -NDT & QUALITY SERVICES HSE


MANAGEMENT POLICY AND ITS APPLICATION

3.5 General

The CEO of BELMONT ASIA -NDT & QUALITY SERVICES,


acknowledge their responsibility for the health and safety of
BELMONT ASIA -NDT & QUALITY SERVICES personnel and
damage to property and the environment which may result from the
company's activities.

BELMONT ASIA -NDT & QUALITY SERVICES policy is to


minimise the potential for accidents and incidents, as far as this is
reasonably practicable.

This objective will be achieved by compliance with BELMONT


ASIA -NDT & QUALITY SERVICES HSE Management System
and existing HSE regulations.

BELMONT ASIA -NDT & QUALITY SERVICES HSE


Management System shall be applied during all phases of the
Project.

BELMONT ASIA -NDT & QUALITY SERVICES will implement


and maintain a project specific HSE system to the highest standards
in order to:

 efficiently protect the health of BELMONT ASIA -NDT &


QUALITY SERVICES workforce and of all other personnel
 maintain and improve all safety measures
 protect the environment during all phases of the project.

These objectives will be achieved by:

 Implementation and maintenance of effective plans, procedures


and systems to control risks
 Protecting the environment by minimising wastes, preventing
contamination by spillages and seeking improvements in waste
management and disposal.
 Attempting to identify and eliminate potential safety and
environmental hazards.
 Identifying the root causes of accidents/incidents and
disseminating this information to all Project personnel.
 The regular verification of the effectiveness of HSE measures
by the performance of regular safety and environmental audits.

3.6 BELMONT ASIA -NDT & QUALITY SERVICES Health


Safety Guidelines

Health and Safety at work is an important aspect of BELMONT ASIA -


NDT & QUALITY SERVICES international business activities.
Consequently the Senior Partners of BELMONT ASIA -NDT &
QUALITY SERVICES support all measures to continuously improve
work safety and the health of the workforce.

Accidents and incidents can be totally or at least partially avoided if


risks and dangers are assessed and countered by an organised
implementation of safety measures.

These include in particular:

 a clear assignment of authorities and responsibilities.

 full attention, without any compromise, to HSE standards, the


use of technical safety installations and personal protective
equipment.
 the analysis of equipment, materials and working methods to
identify potential hazards.
 compliance with CLIENT Contract and BELMONT ASIA -
NDT & QUALITY SERVICES internal specifications and
procedures.
 the qualification and continuous training of all personnel.
 investigation of accidents and incidents in order to identify the
root cause(s) and cascade this information throughout the
organisation.

The promotion of health and safety improvement is one of the


predominant tasks of each BELMONT ASIA -NDT & QUALITY
SERVICES employee. The prevention of accidents and incidents on
the basis of efficient measures is an indispensable proof of
qualification.

However, it is only through planned co-operation of all employees


that these guidelines can be successfully applied in practice in order
to further enhance work safety and health issues of our workforce.

4.0 SITE HSE ACTION PLAN

4.1 HSE Objectives

The primary objectives of BELMONT ASIA -NDT & QUALITY


SERVICES HSE activities are to eliminate negative impacts on the
health and safety of all Project personnel and to optimise
environmental protection measures, in so far as this is reasonably
practical.

Individual HSE objectives include the following actions or


activities:

 To demonstrate that all hazards that carry the potential to cause


harm, including ill health, injury, damage to property or the
environment, or BELMONT ASIA -NDT & QUALITY
SERVICES reputation, have been identified.
 That appropriate controls and recovery methods are in place to
eliminate, or at least, minimise identified risks.
 To prevent the occurrence of accidents and incidents during
project execution.
 To maintain and improve safety and environmental standards
by the constant review and revision of procedures.
 To encourage the free flow of information to all levels of the
workforce with regard to the causes of accidents and incidents.

The HSE objectives for the CLIENT Project will be communicated


to all personnel with action taken on site by line management.

4.2 Site Safety Meetings

4.2.1 Toolbox Meetings

Weekly toolbox meetings shall be conducted by Inspectors


on site. The topics for toolbox meetings will address HSE
issues such as housekeeping, eye protection, lifting and
handling, etc.

Individual, CLIENT and BELMONT ASIA -NDT &


QUALITY SERVICES personnel are expected to participate
in these meetings by offering suggestions regarding safe
working practises and to highlight any unsafe situations
and/or actions which they have observed.

Information concerning the findings of accident/incident


investigations will also be disseminated during these
meetings.

4.3 Accident / Incident Investigation and Follow Up

Reporting
All accidents, incidents and near misses will be reported
immediately to CLIENT and will be routinely investigated using
approved procedures, according to the CLIENT requirements.

Results
The results of all accident or incident investigations shall be
discussed and reviewed by BELMONT ASIA -NDT & QUALITY
SERVICES Project Management and preventive measures will be
implemented.

Dissemination of Results
Accidents/incidents and preventive actions recommended will be
discussed during weekly toolbox meetings.

4.4 Vehicles

All vehicles allocated to the Project shall be suitable for the work
for which they are deployed.

4.5 Personal Protective Equipment (PPE)

PPE is provided to all personnel where it is required. The use of


head, foot, eye and hearing protection is mandatory for all persons
working on site. Additional PPE, specific to construction activities,
will be provided. The use of PPE issued to personnel is mandatory.

4.6 Reporting of Accidents and Incidents

Accidents and incidents shall be reported immediately by workers to


their immediate supervisor and by supervisors to Project
Management. Representative shall ensure that CLIENT is notified
immediately of any accidents and/or incidents.

5.0 GENERAL GUIDELINES, OPERATIONAL SAFE PRACTICES


AND STANDARDS

5.1 Safe Working Practices

Safe working practices will be explained to employees during


weekly toolbox meetings to supplement the safety guidelines issued
to all personnel and give additional information identifying hazards
and precautions to be taken for construction activities.

5.2 Health

Emergency telephone numbers of physicians, hospitals and


ambulance services shall be prominently posted in all site offices.
Crew foremen, Discipline Supervisors and Sub-suppliers
supervisors shall made aware of this information and instructed as to
how to call for immediate assistance. Maps of the locations of
medical facilities will also be displayed.

5.3 First Aid

Provisions for prompt medical care and first aid services for every
employee shall be made available by CLIENT on site.

5.4 Housekeeping and Safe Access

Good housekeeping is one of the prerequisites of accident


prevention and shall be of primary concern to all employees.

1. During the course of job all work areas must be kept clean of
scrap and debris of all kinds.
Rubbish, debris, waste and useless materials constitute fire and
accident hazards and shall be removed from work areas as it
accumulates.

Tools and equipment which are no longer required should be


removed from the work area.

6.0 ENVIRONMENT

6.1 Objectives
BELMONT ASIA -NDT & QUALITY SERVICES Management is
committed to comply with the guidelines of the CLIENT Project
and local regulations in all matters of environmental protection
related to the execution of the Project.

BELMONT ASIA -NDT & QUALITY SERVICES will identify


potential environmental problems such as accidental spills, handling
of combustibles, chemical substances etc. in order to avoid
incidents.

Waste management throughout the project is of prime importance. It


is the defined objective of BELMONT ASIA -NDT & QUALITY
SERVICES to:

 reduce the generation of waste


 handle and dispose of waste in accordance with rules and
regulations
 minimise the overall impact on the environment.

6.2 Practice

Environmental protection measures will be planned and taken in


accordance with this HSE Plan. Individual practical measures will
de directed at prevention rather than reacting to incidents. The
following practical steps will be followed:

1. Waste oil etc., from activities will be disposed of in the


allocated chemical disposal area (if such disposal areas are
allocated by CLIENT).

2. Litter and waste will be handled and disposed off in compliance


with approved CLIENT procedures.

Waste Management
Waste generated during the activities shall be disposed off
through existing waste facilities available to BELMONT
ASIA -NDT & QUALITY SERVICES and subcontractors.

Discipline Supervisors and Subcontractor Supervisors will


be responsible for corresponding actions.

Any accidental spillage shall be contained and cleared as


soon as possible. Where necessary, the area of spillage shall

be cordoned off and hazard signs be posted. Spillages of


hazardous liquids are to be reported.

Working on Site

Any wastes in the result of Job or work, shall be only


disposed off through use of the appropriate facilities.

7.0 COMPLIANCE WITH REQUIREMENTS AT CONTRACT


COMMENCEMENT

7.1 Contract Requirements

Compliance with CLIENT Safety Guidelines and Regulations is a


Contract condition. All site employees will be issued with a copy of
CLIENT HSE policies and instructed to comply with the
requirements stated therein.

7.2 Training

BELMONT ASIA -NDT & QUALITY SERVICES Representative


will assess the need for additional safety and environmental training
in the context of the CLIENT requirements and arrange for any
training to be carried out.

COMPLIANCE WITH ENVIRONMENTAL SPECIFICATIONS

Because of the significant importance to use natural resources in an


environmentally sound manner while protecting the health and safety of our
employees and the public, we herewith repeat the BELMONT ASIA -NDT
& QUALITY SERVICES guiding environmental principle, i.e. our
dedication to continuous efforts to improve the compatibility of our
operations with the environment while economically supplying high quality
products and services to our clients.

Since environmental considerations continue to be a priority and key element in


our planning of inspection operations, our related logistics and processes, the
environmental protection measures will be in full compliance with Project
environmental specifications and requirements as well as with the laws, rules and
regulations of the Government of Pakistan.
