AFFIDAVIT OF TWO DISINTERESTED PERSONS

AS TO DISCREPANCY IN NAME

We, NIEVA V. ANGELADA and MARIA C. ILLUSTRISIMO, both of legal age, Filipinos,
both residents of Barangay Tiniguiban, Puerto Princesa City, Palawan, after having
been duly sworn to in accordance with law, hereby depose and state the following:

1. That we are close friends with SALVADOR VALCUEBA SACLET, by virtue of our
common hometown as well as being colleagues at the Provincial Capitol;

2. By reason of our close relations, we are also known to the parents of

SALVADOR, his mother being MRS. CERIACA VALCUEBA, and his father MR.
GAUDENCIO SACLET. The same names indicated in SALVADOR’s Birth
Certificate, herein attached as Annex “A”;

3. Despite the above information, and due to possible inadvertence, the name
CIRIACA BALCUEBA is indicated as the mother of Salvador in his Marriage
Contract (Annex “B”), further causing Salvador to use the changed middle
name ‘Balcueba’ in several of his legal or formal documents upon marriage;

4. From the foregoing, we are now executing this affidavit to attest to the fact
that the name CERIACA VALCUEBA and CIRIACA BALCUEBA refer to one and
the same person – and to the same effect, the name SALVADOR BALCUEBA
SACLET and the name SALVADOR VALCUEBA SACLET as appearing
respectively in the above documents refer to one and the same persons;

5. Furthermore, we are executing this affidavit to in the interest of Mr. Salvador

Saclet with full knowledge of its legal consequences, without consideration of
any promise or future reward.
