The Alumni of MSU-Main Campus may now request Transcript of Records or any school documents on-line,
provided the requestor had been cleared from all deficiencies. The requesting party must be the original
owner of the Transcript of Records or any school document/s requested.

Here are the following steps in requesting school documents ON-LINE:

1. Fill out the Request Form;


2. Scan the accomplished Request Form and submit it to this e-mail address
(alumnimsumainregistrar@gmail.com);
3. Once request is approved, send the payment thru Money Transfer (Palawan Express
Padala);
4. Send a scanned copy of the proof of Payment (deposit slip)and send it to this e-mail
address (alumnimsumainregistrar@gmail.com);
5. Wait for the confirmation via email or call if the request document/s was sent to your
mailing address.

School Document/s Fees


 TRANSCRIPT OF RECORDS P50.00 per page;
 CERTIFICATION OF GRADUATES P50.00 per page;
 AUTHENTICATION P20.00 per set;
P50.00, plus P50.00 per page
 HONORABLE DISMISSAL of the official TOR to be issued
upon request of the
requesting school; and a fee
of P125.00 for its mailing
 CGPA P50.00 per page;

PROCEDURE WHEN SENDING PAYMENT

STEP 1 Go to any PALAWAN EXPRESS PADALA near you;

STEP 3
Once payment has been made, send a scanned copy of the Proof of
Payment (Receipt) to this e-mail address:

Conditions and Reminders:

Send the exact amount.

After we received your request/s, non-payment within 5 working days, and non-
compliance under Step 3 shall mean cancellation of your request/s.

Since this is a newly launched mode of request initiated by the MSU-University Registrar’s
Office, some PALAWAN BRANCH may not be aware that the Palawan Pawnshop Inc’s Head
Office allows the receiver’s first name be under the name of “MSU MARAWI” and the receiver’s
last name is “REGISTRAR”, the sender should explain to the teller or to the Palawan personnel that
this arrangement was already communicated to PALAWAN-MSU Marawi Branch by our Office
and approved the same by the Palawan Pawnshop Inc’s Head Office;

Please take note of the (DESTINATION BRANCH: CCC) when you send payment to
Palawan Express- this is very important.

The requesting party has the option to cancel his/her request, PROVIDED payment has not
been made thru money remittance (Palawan Express Padala).

Processing time of requests for academic records commences only upon presentation of
the proof of payment (scanned copy) for the document/s requested.

Processing Period: 5-15 working days.
