PETITION

Petitioner and unto this Honorable Court, most respectfully states THAT:

1. Petitioner is of legal age, having been born on Decmber 5, 1982,

single, Filipino citizen and presently residing at Brgy. Mag-ata, Tomas
Oppus, Southern Leyte;

2. Petitioner has been admitted to the practice of law in the Philippines as

shown by the Certificate of Membership in the Philippine Bar, which
contains the Petitioner’s Roll of Attorney No. 70454. A copy of the
Certificate of Membership in the Philippine Bar is hereto attached and
marked as Annex “A” and made as integral part hereof;

3. Petitioner is likewise a bona fide member in good standing of the

Integrated Bar of the Philippines as shown by his Certificate of Good
Moral Character. Attached and marked herewith as Annex “B” is the
Certificate of petitioner’s good moral character issued by Atty. Vincent
Abril U. Rodas and Atty. Aemilda B. Madredijo, President and
Secretary, respectively, of IBP Southern Leyte Chapter. A photocopy of
his IBP Receipt showing his IBP No. 043110 is hereto attached and
marked as Annex “C” and made as integral part hereof;

4. Petitioner has also paid the Professional Tax Receipt due him as
evidenced by PTR No. 6380624, dated 05/29/2018, a photocopy of
which is also attached herewith and marked as Annex “D” and made
as integral part hereof;

5. Petitioner has all the qualifications and none of the disqualifications for
the Office of the Notary Public as evidenced by a photocopy of his
certificate issued by the Office of the Bar Confidant that there is no
pending administrative case against him is also attached herewith and
marked as Annex “E”;

6. Petitioner has never been charged nor convicted of any crime involving
moral turpitude, as evidenced by a photocopy of his clearance showing
no derogatory records issued by the National Bureau of Investigation is
also attached herewith and marked as Annex “F”;

7. Petitioner has no commission as notary public in any other jurisdiction;

8. Petitioner has attached hereto in a separate sheet of paper specimen

of his official signatures and marked as Annex “G” and made an
integral part hereof;

9. Petitioner has likewise attached thereto his three (3) passport size
photographs each bearing his signature on the bottom part thereof as
Annex “H” and made as integral part hereof.

PRAYER

WHEREFORE, Petitioner respectfully prays unto this Honorable Court

that his petition for Notarial Commission be granted.

VERIFICATION
