Page 1
Rev. 6/05
Objective: (include methods of assessment) (Fall)
I will create strategies to intervene with struggling students to increase their learning in math
and reading as measured by 80% of students will move up to levels 3,4, and 5 on their I Ready
profiles. Methods of assessment include observations of students, and future diagnostic I Ready
assessment data.
Objective: Degree of Attainment (Spring)
According to the data, I have not met this objective. However, I have created small group
intervention for profile 1 students in reading and math. They get intervention three times a week in
class. I have also implemented small group intervention in class two times a week for profile 2
students in reading and math. In addition, I provide after school intervention for my profile 1 students
three times a week. Based on the I Ready Reading Assessment data, in the fall there were 11
students in profile 1 and 3 students in profile 2. The latest diagnostic data shows that there are 8
students in profile 1 and 4 students in profile 2. The IReady Math Assessment data shows there were
6 students in profile 1 and 9 students in profile 2. The latest data shows that all students from profile 1
moved up to profile 2 ( and 2 moved from profile 2 to 5).

Summary:
Dale has shown significant growth this year in arranging and crating challenging learning
experiences for his students. He has sought out and implemented many new strategies and grasped
the effective use of data.

Areas of Commendation:
Dale is very conscious of each child’s proficiency level and works to support student learning.
Dale has incorporated technology to enhance his lessons. Dale takes teaching seriously.

Next steps:
•Continue to learn and implement new strategies that fit your teaching style.

Standard 2: Creating and Maintaining Effective Environments for Student Learning

Create physical environments conducive to learning
Engage students in purposeful activities

Page 2
Rev. 6/05
Encourage constructive interaction among students
Maintain a safe learning environment
Treat students fairly and respectfully on consistent basis
Encourage students to participate in making decisions
Encourage students to work independently and collaboratively
Establish clear expectations for student behavior early and be certain they are clearly understood
Effective use of instructional time.

Evaluatee’s Objective(s)
Objective: (include methods of assessment) (Fall)
I will be able to put strategies into place to eliminate negative student behavior as measured by
80% fewer students off task. Methods of assessment are observations of lessons, and observing
classroom physical environment.

Objective: Degree of Attainment (include methods of assessment) (Spring)

I have attained this objective by implementing classroom management strategies such as: the
use of table points, attention signals (calling a timeout by making a “t” with their hands, creating a
social contract with the students, weekly goals with incentives (fun Friday). Physically, I have situated
my classroom in a more effective way to manage the classroom as well. I have also put posters,
charts, and children’s work on the walls to create a positive environment for the children. We have our
“Good Things” morning meeting where students tell me something good about their lives. In addition,
an “absolute” schedule and routine has been created, and time management has become more
effective, making less behavioral issues possible and more students on task. There are only four
students that have behavioral issues now compared to about 75% of the class in the fall.

Evaluator’s Response To Standard 2

x
Exceeds District Standards Meets District Standards Unsatisfactory

Summary:
Dale was handed a difficult situation when he took over the class seven weeks into the school
year. He has worked diligently to create a cohesive learning envirornment where every student
understands and works to obey class rules and where everyone feels safe both physically and
emotionally.

Areas of Commendation:
Dale seeks out management strategies and tries them out to find the best fit for himself and his
class. He has integrated effective strategies. The class has made a full 180 degree turn under his
command.

Standard 3: Understanding and Organizing Subject Matter for Student Learning
Exhibit strong working knowledge of subject matter and student development
Organize curriculum to facilitate student understanding of central themes and concepts
Interrelate ideas and information across curricular areas
Use knowledge of student development, subject matter, resources, and strategies to make subject matter
accessible to all students

Evaluatee’s Objectives
Objective: (include methods of assessment) (Fall)
I will be able to organize curriculum to facilitate student understanding of central themes and
concepts as evidenced by lesson plans and student work samples. Methods of assessment include
observations and portfolios of lesson plans.

Objective: Degree of Attainment (include methods of assessment) (Spring)

I have attained this objective by mentorship from my co workers and making use of the
curriculum resources. For example, I frequently use technology in my lesson plans (The interactive
GOMath! digital resource). I have my broad based yearly pacing guide that aids me in planning out
each month. I make copies of all my worksheets at least two weeks in advance. Then I use Google
Docs to create my weekly lesson plans. Each night I review the lessons to be taught and make any
corrections to my next day lessons. I have been able to effectively interrelate ideas across curricular
areas as well. For example, I recently created a language arts lesson on inflectional endings that
included an activity in which the students created a cube ( they are learning about 3d shapes as well)
with different base words that they must change to the -ing and -ed spelling. This connected math
with language arts as well as art. Based on the students work, it is determined they responded very
well and internalized the information from the lesson.

Summary:
Dale has immersed himself in 2nd grade standards and curriculum and pacing. He has a good
handle on lesson design, pacing, and flow. He uses a variety of assessments and monitors the effects
and success of his lessons on student achievement.

Areas of Commendation:
This has been an extreme learning curve this year for Dale, and he has risen to the challenge to
understand the ins and outs of 2nd grade teaching.

Follow curriculum sequence that reflects grade-level expectations
Modify instruction according to student engagement and achievement

communications, he has been able to address academic and social-emotional learning with each of
his students.

Areas of Commendation:
Dale has taken the time to know his students as individuals.

Next steps:
Keep it up!

Standard 5: Assessing Student Learning

Establish and clearly communicate learning goals for students
Collect information about student performance from a variety of sources
Page 7
Rev. 6/05
Involve students in self-assessment
Adjust learning as necessary
Communicate progress with students and parents

Evaluatee’s Objective(s)
Objective: (include methods of assessment) (Fall)
I will be able to effectively collect information about student performance from a variety of
sources as evidenced by portfolios of student data. I will be able to effectively communicate progress
with students and parents. Methods of assessment include diagnostic data from testing as well as
formative and summative tests and student portfolios.

Objective: Degree of Attainment (include methods of assessment) (Spring)

I have attained this objective and am able to effectively assess and communicate progress with
both students and parents. For example, before every concept I teach, I pre assess the students to
find out what they already know about the subject. At the end of every lesson, I create formative
assessments called “Exit Tickets” in which I gather data about what each student knows so I can pull
struggling students for a small group re-teach if necessary. During the lesson, I love to make use of
the small white boards that students have to quickly assess which students are getting the concepts
and which are not. Then during independent practice, I know which students to assist. Summative
assessments are graded as soon as possible and sent home to parents after the student and I
conference about their scores. I make use of performance tasks as well to assess my students, using
a grading rubric. This allows for me to see how the child is thinking rather than just filling in a letter on
an assessment. In addition, it measures students skills based on authentic tasks such as activities,
exercises, or problems that require students to show what they can do. AR quizzes are available for
the parents to view online as well for reading comprehension. The students also have diagnostic
testing periodically throughout the year using the STAR READING Assessment and the IReady
program. This data is discussed with parents. I communicate with parents using Class Dojo to update
them on their child’s progress in my classroom. Furthermore, I send home progress reports each
trimester to inform students and parents of progress.
