1. Quality Assurance Procedure (QAP)


1.1 Purpose
Fortress Infracon Limited (formerly known as Fortress Infrastructure Advisory
Services, Division of Fortress Financial Services Limited) with organization structure
founded on a well-defined quality policy. Quality Assurance Plan covers all aspects
of field studies, investigations, design, checking procedures and economic/financial
analysis. The quality assurance plans / procedures for different field studies,
engineering surveys and investigation, design and documentation activities are
presented in this report.

1.2 Background
The QAP with particular reference to the project has been prepared to be consistent with
Quality System manual of Fortress Infracon Limited and compliant with ISO 9001:2008,
and it provides for a comprehensive and systematic quality assurance programme in
conformity with the declared quality policy of Fortress Infracon Limited. The QAP
explains comprehensively the scope of services along with quality procedures and
plans.

The Quality Assurance Plan by including procedure and methods for surveys and
investigations, design activity and checking procedures (three levels) to ensure quality
of deliverables. The QAP prepared include details of people who are responsible for
carrying out / preparing and checking / verifying various activities forming part of the
study.

The quality assurance plan / procedure for field studies and for design and
documentation activities are presented in the report. The QAP is based on the
Consultant’s Quality Assurance System. The TEAM LEADER nominated for the project
formulates and monitors the quality system for this project and the team leader will
assure the effective implementation of this system. The Team Leader will ensure that the
associates working for this assignment adhere to this Quality Assurance Plan.

1.3 Scope of Services for the assignment


The scope of services includes but is not limited to the following tasks:
 Review of all available reports and published information about the project road
and the project influence area;
 Environmental, poverty and social impacts, including those related to cultural
properties, indigenous people natural habitats, involuntary resettlement etc.;

 Public consultation, including consultation with Communities located along the


road, NGOs working in the area, other stake-holders and relevant Govt. depts. at all
the different stages of assignment (such as inception report stage, interim report
stage, detailed project report (Part-I) stage and detailed project report (Part-II)
stage;
 Detailed reconnaissance;
 Identification of possible improvements in the existing alignment and bypassing
congested locations with alternatives, (please note that the project design does not
envisage any land acquisition, hence such elements shall not be included for
further evaluation, design, costing and analyses); however LA plan for such
location from safety consideration be prepared and submitted to within two
month from date of work order so that land acquisition, if any, required to be
completed as soon as possible.
 Traffic studies including traffic surveys and Axle load survey and demand
forecasting for next thirty years; detailed traffic study along all the arms of the
intersection including OD survey, traffic pattern during the peak hours, detailed
traffic and axle load survey, demand forecasting for next 10, 20, 30 years and scope
of peak hour congestion at the intersection by utilizing the existing local roads;
 Inventory and condition surveys for road;
 Inventory and condition surveys for bridges, cross-drainage structures and
drainage provisions;
 Detailed topographic surveys using LIDAR or similar technology;
 Pavement investigations;
 Sub-grade characteristics and strength: investigation of required sub-grade and
sub-soil characteristics and strength for road and embankment design and sub soil
investigation;
 Identification of sources of construction materials;
 Detailed design of road, its x-sections, horizontal and vertical alignment and design
of embankment of height more than 6m and also in poor soil conditions and where
density consideration require, even lesser height embankment. Detailed design of
structures preparation of GAD and construction drawings and cross-drainage
structures and underpasses etc.
 Identification of the type and the design of intersections;
 Design of complete drainage system and disposal point for storm water;
 Value analysis/value engineering and project costing;
 Economic and Financial analyses;
 Contract packaging and implementation schedule;
 Strip plan indicating the scheme for carriageway widening, location of all existing
utility services (both over- and underground) and the scheme for their relocation,

trees to be felled and planted and land acquisition requirements including


schedule for LA: reports documents and drawings arrangement of estimates for
cutting of trees and shifting of utilities from the concerned department;
 To find out financial viability of project for implementation under Hybrid Annuity
model and suggest the mode on which the project is to be taken up.
 Design of toll plaza and identification of their numbers and location and office cum
residential complex including working drawings.
 Design of weighing stations, parking areas and rest areas.
 Preparation of detailed project report, cost estimate, approved for construction
drawings, rate analysis, detailed bill of quantities, documents for execution of civil
works through budgeting resources.
 Any other user oriented facility enrooted toll facility.
 Tie-in of on-going/sanctioned works of MORT&H/PWD/PWD/Govt. of
Maharashtra /other agencies. Preparation of social plans for the project affected
people as per policy of the lending agencies/ Govt. of India R & R Policy.

While carrying out the field studies, investigations and design, the development plans
being implemented or proposed for future implementation by the local bodies, should
be taken into account. Such aspect should be clearly brought out in the reports and
drawings.

The consultant shall study the possible locations and design of toll plaza, wayside
amenities required and arboriculture along the highway shall also be planned.

The local and slow traffic may need segregation from the main traffic and provision of
service roads and physical barrier including fencing may be considered, wherever
necessary to improve efficiency and safety.

1.4 Organizational Setup


Fortress Infracon Limited Head Office is located in Mumbai, where fully equipped design
departments are present with all highway study and design activities. The organization
structure of Fortress Infracon Limited comprises of Base organization and Project
organization.

1.4.1 Project Organization


The project organization is created for all individual assignments / projects. The
Directors of the company in consultation with each other will create this organization.
A project organization for this assignment is presented in Figure 1-1.

Since the project involves more than one department, one Lead Expert (LE) from each
department will be appointed.

1.4.2 Team Leader


Team Leader will lead the team for the overall performance of the project. The
responsibilities of the Team Leader are:
 To define responsibilities and delegate authorities for the Project Engineers (PE).
 To prepare, implement and maintain the Project Quality Plan for the project
in accordance to the Quality System.
 To supervise the project’s quality management including identification and
registration of quality problems and to ensure resolving them with the TEAM
LEADER.
 To identify suitable and qualified personnel for undertaking project tasks.
 To guide and monitor the entire project team towards the delivery of the project
as per the requirements.
 To interface with the team members working on the project for timely completion
and delivery of client’s requirements.
 To ensure that the project team adheres to all procedures for planning, document
control, review and check.

1.4.3 Leader Experts (LE)


Leader Experts are the lead staff leading with aspects related to different
disciplines of Fortress Infracon Limited. The responsibilities of the Leader are:
 To achieve project milestones as defined by the Team Leader (TL)
 To define and delegate tasks and authorities to Project Engineers (PE) of
the respective disciplines.
 To carry out the project work within respective Department in collaboration
with one or more Support Staff.
 To report all technical matters concerning the project to the Team Leader
 To own responsibility for the quality of the work carried out within the LE’s team
/ Department with respect to cost, time and technical elements.

1.4.4 Project Engineer (PE)/Support Staff (SS)


The responsibilities of the Project Engineer are:
 To carry out tasks defined by the LE
 To seek guidance and concurrence of the TL / LE in all technical decisions and
associated with the tasks assigned to him.

2. Project Quality System


2.1 Internal Audits & Checks
A Quality Audit is a systematic, independent and documented process for obtaining
audit evidence and evaluating it objectively to determine the extent to which Quality
Assurance Plan adheres to.

The main objective of conducting frequent internal audit is to ensure consistent quality
of deliverables, to make continual improvement and achieve better client satisfaction
in terms of the deliverables.

2.2 Planning and Implementation of Internal Quality Audit


The auditor makes a short introductory explanation of the audit and is carried
out as a meeting between the auditor and the Project Leader / Engineers. During the
audit, relevant points are noted on the checklists and the same will be informed to the
team upon the completion of audit. The auditor mainly looks in to the following issues:
 Non-conformities from the established Fortress Infracon Limited QMS/ QAP
 Recommendations proposed to improve the quality management for the project
 Conclusions regarding the efficiency of the quality management for the project
 Non-conformity is departure or absence of one or more quality characteristics or
Quality System elements from specified requirements. When non-
conforming services are discovered the following measures are adopted to limit
the consequences:
o Involved parties inside and outside the project organization are
informed of the discovered non-conformities.
o Non-conforming documents are identified, recalled and marked accordingly
in order to avoid unintentional use.

2.3 QAP for Field Surveys and Investigations


The project involves various surveys and field investigations such as topographic
survey, traffic survey, geotechnical survey, environmental and Inventory survey.
Quality assurance procedures for these surveys are given below:

2.3.1 Material Investigation


A) Scope
The activities will include:
 Identification of potential sources (including use of fly-ash/slag), quarry sites
and borrow areas.
 Estimate quality and quantities of various construction materials available for
collection of samples and conducting relevant laboratory tests.

 Evaluation of test results and asses the suitability thereof for incorporation in the
various works and making recommendation on the use of the materials from
different sources based on techno-economic considerations.

B) Manpower and Responsibility


The Materials cum Geotechnical Engineer assisted by an Engineer will visit all the
potential quarries and material sources, and based on his visual inspection he will
select the sites from where samples will be collected for laboratory testing.

C) Equipment
The Equipment will include hand tools like crowbars, spades, wedges etc. for collecting
representative samples. The samples will be packed in suitable bags, labeled and sent
to laboratory of the Consultants for testing. Samples of stone aggregates will be
collected from existing crushers using the stone from the respective quarries.

D) Applicable Standards/Codes
Sampling and testing of borrow soil, granular material for sub-base, stone aggregate
and sand will be as per relevant latest IS/IRC standards and Specifications.

E) Regulatory Requirements
The samples will be collected after obtaining necessary authorization from the PWD
and concerned officials.

F) Safety/Environmental Considerations
Environmental and social standards of World Bank / ADB / GOI will be taken into
account while choosing the prospective quarries and material sources. The
investigating and sample collection teams will be provided with safety gears such
as gum boots, hats and dust protecting goggles. Any hole made for sampling will be
refilled and the area made good before leaving the site.

G) Data Verification
The Materials cum Geotechnical Engineer will verify that the samples collected are
representative and that they have been collected in sufficient quantities from all the
potential quarry and material sources. The Materials cum Geotechnical Engineer will
check all the test results received from the laboratory for compatibility and make an
evaluation for suitability.

H) Work Procedure
i) Collection of Secondary Data
To start with, secondary data as follows will be collected from the concerned R&B /
other sources as appropriate.
 Location maps of operating quarries and material sources
 Technical report on survey and evaluation of locally available materials
 Land use / Land cover map
 Geological map of the area
Discussion will also be held with Consultants / Contractors working on nearby
projects to ascertain the sources used by them, the cost of extraction, etc.

ii) Collection of Sample


Each quarry / material source will be visited by the Materials cum Geotechnical
Engineer, and the available material examined in detail. The location of the source,
the approachability, ownership, any environmental and social restrictions, volumes
of material available for extraction, any special installation/Equipment required for
extraction, visual assessment about suitability, and other relevant information will
be recorded.
Samples will be collected from each source adopting standard sampling procedures.
These will be packed in suitable bags, labeled and sent to the laboratory for testing.

iii) Laboratory Testing of Borrow Soils and Granular Materials


The samples of materials for use in embankments, subgrade and granular sub-base
will be subjected to the following tests:
Determination of water content -As per IS: 2720 (Part 2)
Grain size analysis -As per IS: 2720 (Part 4)
Liquid and Plastic Limit -As per IS: 2720 (Part 5)
Compaction Test (Heavy Compaction) -As per IS: 2720 (Part 8)
Determination of CBR -As per IS: 2720 (Part 16)

iv) Review and Verification of Test Results


The Materials cum Geotechnical Engineer will review all the laboratory test results
for consistency and compatibility. For example, for soils and other granular materials,
the quality given by classification and maximum dry density results will be compared
against that given by CBR. For rock samples, the specific gravity, water absorption
and the aggregate impact value will be checked against the petro-graphic
classification of the rock. For any inconsistencies or where the results are in doubt,
the tests will be repeated. If the material test shall be carried through a sub-
consultant, same procedure will be applicable after appointing a sub-consultant.

2.3.2 Topographic Survey


Topographic survey captures true to ground realities using precision instruments like
Total Stations and Auto Levels, and bringing out data in digital form (x, y, z format) for
developing digital terrain model (DTM). It captures all existing physical features,
including utility poles, trees of girth greater than 0.3 m, etc., within the survey corridor
which should be compatible with the widening requirements subject to a minimum of
30 m on either side of the final road center line.

Additional surveys at bridge sites for hydraulic calculations and for all arms of crossing
roads at intersections. Also, detailed topographic surveys along approved alignment of
proposed bypasses. Longitudinal sections at 25 m or at closer intervals on curves, and
cross-sections covering the full width of survey corridor at 50 m interval. Also grid
leveling on existing carriageway as appropriate for determining the thickness of profile
correction course.

Fixing horizontal and vertical control points with concrete pillars. The BMs with levels
drawn from GTS benchmarks shall be fixed at 250 m intervals. The Reference / Bench
Mark pillars shall be of 15 cm x 15 cm x 45 cm sizes in RCC grade M 15 with nail fixed
in the center of the top surface. The pillars shall be embedded in CC of M 10 grade upto
a depth of 30 cm, and the balance 15 cm shall be painted yellow.

A) Manpower Responsibilities
The topographic survey may be carried out by an experienced sub-consultant (vendor).
The vendor will be selected in accordance with the discussions carried out within the
Project Management Team. The Companies' Survey Engineer shall be responsible for
the adequacy, coverage and correctness of the surveys.

B) Equipment
 Differential GPS Equipment/LiDAR Technology
 Electronic total stations with angular accuracy of +/- 5sec.
 Auto Level having accuracy of +/- 2.5 mm/km.
 Other items like measuring tapes, ranging rods etc. as required.

C) Applicable Standards Codes


The work procedures and quality plan prepared with reference to IRC: SP 19 - 2001,
IRC SP 13 - 2004 (in respect of surveys for rivers / streams) and current international
practices.

D) Regulatory Requirements
The work will commence as per the activity schedule.

E) Safety Consideration
The survey personnel will wear luminous yellow jackets. The instrument stations will
be established outside travelled way to avoid conflict with moving traffic.

F) Data Verification and Accuracy


At the end of each day's work, the collected data will be downloaded into the
computer and the Survey Engineer will verify the correctness and completeness of the
survey data. The level of accuracy to be ensured shall be as follows:
 Better than 1: 20000 for all distance measurements
 The closing error for level measurements shall be within +/- 6 (K) 0.5 where K
is the distance in km.

G) Survey Procedure
i. Fixing of Traverse Stations
Location of traverse stations shall be selected on the following basis:
 It should be located on the edge of the road so that traffic is not disturbed while
the survey IS in progress
 It should as far as possible be near to a Kilometer/Hectometer stone, so that it
can be identified easily on field.
 Traverse stations should be located such that last station signal bottom and next
station signal bottom are clearly visible from the occupied station. This distance
can be increased when there is no obstruction to vision up to max 300 m. This
distance can be decreased in case of horizontal and vertical curves.
 Traverse will be done by Total Station having angular measurement accuracy of
+/- 1 sec. The instrument shall be calibrated at the start of the work.

ii. Location of Bench Marks


These should be located at interval near a Kilometer / Hectometer Stone
 It should be as per the specification provided.
 It should be located based on the widening requirement so that these remain for
future reference during construction
 The RL and BM No. will be marked on the pillar with red paint.

iii. Fixation of Bench Mark Levels


All levelling for establishing Bench Mark are to be carried out as per method
adopted by Survey of India DT levelling and specified as under. All levelling are to
be carried out from GTS Bench Mark with Auto Level having accuracy +/- 2.5
mm/km and closed against another GTS Bench Mark. In absence of GTS BM, to and
fro DT levelling is to be adopted.
 Before starting the work machine should be calibrated
 Intermittently during leveling, collimations are to be checked and adjusted
whenever collimation error exceeds 1 mm.
 Two levelling staffs to be used and fixed with circular babble during levelling i.e.
fore staff and back staff.
 Staff is to be held exactly vertically as per as possible on an iron shoe except on
pillars or any other permanent structures and the fore and back staffs are to be
placed exactly at the middle to nullify the collimation error.
 All readings are to be taken with 3 hairs and mean of the three are to be averaged
for calculation of rise and fall. All rise and fall are to be observed twice.

iv. Setting up Instrument on Station


The site-in-charge shall inspect all instruments to verify that no damage has occurred
in transit. The instrument shall be then set up as follows:
 Set up total station over station and measure height of instrument.

The Vertical Collimation shall be checked on the First station for each day in the
following manner:
 In face left sight to a distant well-defined object and record the vertical circle
reading on the booking sheet
 In face right sight the same object and again record the vertical circle add the two
readings and subtract 360°
 This difference is twice the vertical collimation error
 This error should be less than 20" to maintain the required vertical accuracy
 Check that the prism constant is correct for the prisms being used for the traverse
/detail survey

 Place a mark about 10 m from the station and measure the distance with the 30
m tape
 Take a reading to the prism at this point and ensure the horizontal distance is
within 5 mm of the taped distance

v. Taking Traverse Readings


If using a Prism pole the chain man will ensure that he uses an extra pole to brace the
prism pole whilst taking the readings. The extra pole should be held away from
the line with the Total station to allow the Surveyor to be able to see the bottom of
the Prism pole to set his bearing. Two sets of horizontal angle will be measured
in face left and face right. All horizontal angles of offset points are to be measured
in face left only.

vi. Taking Detail Survey Readings:


After traverse readings, read and record an angle to a reference object other than a
station. This angle can be used to check your orientation if the instrument is
disturbed. This object should be checked back every 50 readings
 Chalk marks shall be made on the road edge every 25 m on straights or very
gentle curves, or 5 to 10 m on sharp curves to align the string readings. The
survey should extend to within 2 m of the previous surveyor half way to the next
station.
 The entire topographic survey from a station should proceed in one direction
from left to right or right to left.

H) Survey by using LiDAR Technology


Lidar is a surveying method that measures distance to a target by illuminating that
target with a pulsed laser light. The name “lidar” sometimes considered an acronym
of Light Detection and Ranging.

Lidar is popularly used to make high-resolution maps, with applications in


geodesy, geomatics, archaeology, geography, geology, geomorphology, seismology, fore
try, atmospheric physics, laser guidance, airborne laser swath mapping (ALSM),
and laser altimetry. The technology is also used for control and navigation for
some autonomous cars. Lidar sometimes is called laser scanning and 3D scanning, with
terrestrial, airborne, and mobile applications. Lidar uses ultraviolet, visible, or near
infrared light to image objects. It can target a wide range of materials, including non-
metallic objects, rocks, rain, chemical compounds, aerosols, clouds and even
single molecules.

There are several major components to a lidar system:

i. Laser –
600–1000 nm lasers are most common for non-scientific applications. They are
inexpensive, but since they can be focused and easily absorbed by the eye, the
maximum power is limited by the need to make them eye-safe. Eye-safety is often a
requirement for most applications. A common alternative, 1550 nm lasers, are eye-
safe at much higher power levels since this wavelength is not focused by the eye, but
the detector technology is less advanced and so these wavelengths are generally used
at longer ranges with lower accuracies. They are also used for military applications
as 1550 nm is not visible in night vision goggles, unlike the shorter 1000 nm infrared
laser. Airborne topographic mapping lidars generally use 1064 nm diode
pumped YAG lasers, while bathymetric (underwater depth research) systems
generally use 532 nm frequency doubled diode pumped YAG lasers because 532 nm
penetrates water with much less attenuation than does 1064 nm. Laser settings
include the laser repetition rate (which controls the data collection speed). Pulse
length is generally an attribute of the laser cavity length, the number of passes
required through the gain material (YAG, YLF, etc.), and Q-switch (pulsing) speed.
Better target resolution is achieved with shorter pulses, provided the lidar receiver
detectors and electronics have sufficient bandwidth.

ii. Scanner and optics –


How fast images can be developed is also affected by the speed at which they are
scanned. There are several options to scan the azimuth and elevation, including dual
oscillating plane mirrors, a combination with a polygon mirror and a dual axis
scanner (see Laser scanning). Optic choices affect the angular resolution and range
that can be detected. A whole mirror or a beam splitter are options to collect a return
signal.

iii. Photo detector and receiver electronics –


Two main photo detector technologies are used in lidars: solid state photo detectors,
such as silicon avalanche photodiodes, or photomultipliers. The sensitivity of the
receiver is another parameter that has to be balanced in a lidar design.

iv. Position and navigation systems –


Lidar sensors that are mounted on mobile platforms such as airplanes or satellites
require instrumentation to determine the absolute position and orientation of the
sensor. Such devices generally include a Global Positioning System receiver and
an Inertial Measurement Unit (IMU).

The 3D point clouds acquired from these type of scanners can be matched with digital
images taken of the scanned area from the scanner’s location to create realistic looking
3D models in a relatively short time when compared to other technologies. Each point
in the point cloud is given the color of the pixel from the image taken located at the same
angle as the laser beam that created the point.

The LiDAR machine proposed to be used is capable of collecting high density 3D scan
data, photogrammetry imaging and high accuracy total station data. It captures both
high-accuracy total station measurements and true high-speed 3D scans together in a
single instrument providing a higher level of measurement performance than ever
before. The scanning station delivers maximum accuracy, efficiency and detail. The
format for carrying out a) Traversing, b) Leveling and c) Typical cross-section for Levels
is given in the Annexure I.

2.3.3 Traffic Survey


The following survey will be undertaken:
 Classified Traffic Volume Count Survey for 7 days at identified locations.
 Turning Movement Count Survey for 24 hours.
 O-D and Commodity movement survey for 24 hours.

A) Manpower and Responsibility


The surveys will be planned and programmed by the Traffic Engineer of the
Consultant Team who will be responsible for identification of survey locations,
monitoring of the surveys and analysis of Traffic data received from the client.

He will be assisted by another Traffic Engineer, who will be directly responsible for
controlling the field operations and quality / coverage of field data. The counting will
be done by trained enumerators, who will be given an orientation of the project
requirements prior to start of the survey.

B) Equipment
Equipment is involved. The enumerators will be provided with suitably structured
Performa for recording the data for the ATCC (Automatic Traffic Counting and
Classification).

D) Regulatory Requirements
All the survey teams must have a copy of the letter of authority from PWD.

E) Safety/Environmental Considerations
The Traffic volume count survey stations will be located where the road is straight and
more or less level and where adequate shoulder space is available for positioning
the enumerators.

F) Training
All the enumerators will be given an orientation about the objective of the project,
the purpose of the survey and the methodology for filling the performa. The
supervising engineers will monitor survey and will carry out periodic checks on
the counts by enumerators.

G) Data Verification
In order to ensure quality and accuracy in data collection the Supervising Engineer will
carry out independent checks intermittently by himself counting at a location away
from the survey station for 15 minutes and check with the data collected by
enumerators.

The summary sheets of each day’s count will be collected by the site engineer from the
surveyors. This data will be used to check consistency of traffic volumes from day to
day of counting with respect to hourly and daily volumes. In case of major variations
exceeding 20% without any valid reason, the survey will be repeated. All the field
survey data sheets will be filled with identification information like date, day, shift
direction, weather condition etc. These will be checked and signed by the supervisor.

H) Work Procedure
The work will be carried out in accordance with the requirements of the relevant
standards and as per the standard procedure of Fortress Infracon Limited for items
not covered by IRC standards. Details of work, procedure by each of the survey type
are presented below:

I) Classified Traffic Volume Count Survey


Location of Traffic count stations will be carefully selected in order to capture all
vehicles using different sections of the project road. Traffic counting will be carried out
manually by trained enumerators, using hand tally. The count data will be recorded at
15-minute intervals for each vehicle group.

J) Turning Movement Count Survey


Location of Turning Movement Count stations will be carefully selected in order to
capture all vehicles from /to all arms of the junction. Traffic counting will be carried
out manually by trained enumerators, using hand tally. The count data will be recorded
at 15-minute intervals for each vehicle group.

K) Origin-Destination survey
Origin-Destination survey will be carried out along with classified traffic volume count
survey. Necessary police permission will be obtained for conducting the survey.
Barricades / cones will be placed for the safe stoppage of vehicles for survey.

2.3.4 Axle Load Survey


Axle load survey in both directions shall be carried out at a suitable location identified
by the Traffic / Highway Engineers.

The survey will be carried out on a random sample basis normally for trucks only (both
empty and loaded trucks) on a normal day for 24 hours. Some sample loadings of buses
plying on the project road shall also be noted. The selection of axle load survey station
shall not include any existing cross drainage structures which have load restrictions
for vehicles passing over it.

A) Manpower and Responsibility


The surveys will be under the direct supervision of the Highway Engineer in
consultation with the Traffic Engineer. He will be assisted by an Assistant Engineer.
Help of local police will be taken to stop and divert the vehicles on to the weigh pads.

B) Equipment
a) Axle load pad
Prior to start of the survey, calibration of the weigh pads and operation of the
recording system will be done through the following procedure:
Place the axle-pad near a weigh bridge
 Measure axle weights on pads
 Add up the total weight of vehicle
 Weigh the same vehicle on a standard weigh bridge.
 Carry out the above for different class and types of vehicles for which axle-loads
are intended to be carried out.
 Develop a graph or equation for calibration of data obtained from axle-load
survey

C) Applicable Codes / Standards


In the absence of any IRC / IS standard on the subject, the work will be carried out
following the Consultants in-house procedure based on the manufacturer's
recommendation.

D) Regulatory Requirements
The survey team will carry with them the following document:
 Letter of authority from PWD to carry out the survey
 Assistance of local police will be taken to stop vehicles and to divert these onto
the weigh pads. The requirement is same for weighing vehicles a weighbridge.
 A temporary sign intimating the conductance of the survey and advising heavy
vehicles to slow down will be posted at about 200 m in advance of the weigh
station in both the directions. Other warning measures as recommended in NHAI
"Pocketbook for Highway Engineers" will also be taken into account.

E) Safety Considerations
The axle load survey station will be located at the same place as that for O-D survey.
This will be located where the road is straight and more or less level and where
adequate shoulder space is available. The location will sufficiently away from
narrow/weak bridges or congested stretches. Preferably, the location should be in an
illuminated place. Otherwise, temporary lights will be used for conducting the survey
during hours of darkness.

Further, flagmen with red flags, and with red lamps during night, will be
positioned for warning public vehicles, and for guiding heavy vehicles to be weighed

on to the weighbridge. Red cones for delineating path of vehicles to be weighed will also
be installed.

F) Data Verification
The data will be recorded by the Engineer on Performa. At the end of the survey,
the manually filled data sheets will be signed on each page by the Engineer and
countersigned by the Highway Engineer as a measure of authentication of the collected
data.

G) Documentation
All the data sheets will be suitably filed and sent to office for data analysis.

H) Work Procedure
The work consists of several activities, and process control checks will be exercised
on these. These include:
 Choice of location of weigh stations
 Correct positioning of weigh pads
 Making arrangement for jerk-free mounting of vehicle tyres onto the pads
 Recording load data on the data sheets and taking the axle load to be twice that
of the weighed wheel load.
 Ensuring that the sample covers all types of commercial vehicles, viz. LCVs,
medium/6 trucks, heavy 3/10 trucks and semi-trailers of different
configurations. A few representative buses will also be weighed. The vehicles
whether they are empty or full would be weighed.
This procedure would be repeated alternately for two sides as it is not possible to
divert traffic from one side to other.

I) Proposed Locations for Axle Load Survey


It is proposed to conduct the axle load survey at the same locations as for the O-D
survey as police help is required for both these activities.

2.3.5 Benkelman Beam Deflection Survey (BBD)


The deflection survey for the existing pavement is to be carried out as per the
following scheme:
Main line testing will be done at 500 m interval for all along the road except those
locations which are badly damaged and proposed for the reconstruction.
For control section testing:
 Homogenous segments are to be determined based on result of pavement
condition survey.

 The deflection testing shall be carried out for each 100m length.
 The total length of homogeneous segment shall not be less than 100m per km.
 The deflection measurement shall be at an interval of not more than 10m.

A) Manpower and Responsibility


Surveys would be carried out under the direction and supervision of Highway
Engineer/Pavement Specialist, and he will be assisted by one Engineer for each team.
The Highway Engineer will direct the sections to be omitted due to realignment, while
the Pavement Specialist will identify very poor/reconstruction stretches where
deflection tests would not give any meaningful results

B) Equipment
The Equipment used will be as follows:
 Standard Benkelman Beam with dial gauge
 Six tonne truck with dual rear wheels
 Banners and red cones for traffic safety
 Glycerol and 0 - 100° C thermometer
 A portable tyre pressure gauge to adjust the tyre pressure to 5.6 kg/cm2, and
hand tools for making holes in the bituminous pavement for temperature
measurement.

At the start of work, the deflection beam will be checked for calibration using filler
gauges of known thickness. The truck will be loaded with bricks uniformly spread over
rear axle and then weighed at a local stationary weigh bridge and the load adjusted in
such a way that the average weight of the rear axle amounts to 8160 kg.

C) Applicable Standards/Codes
“IRC: 81 -1997, Guidelines for strengthening of flexible pavements using Benkelman
Beam Deflection Techniques”.

D) Regulatory Requirements
The survey team will carry with them the Letter of authority from PWD to carry out the
survey

E) Safety/Environmental Considerations
Six safety cones (red and white striped), on the roadside will be positioned when
the deflection measurements are being taken. Besides, banners stating "ROAD TEST -
KEEP DISTANCE" will be posted on the front and rear of the test truck. Further, flagmen

with red flags will be positioned on either side for warning public vehicles from keeping
away from the test truck.

F) Data Verification
Data collected would include:
 Dial gauge reading from the deflection beam
 Pavement temperature measured at a small hole made in the pavement and filled
with glycerol

All the data will be recorded by the Engineer on a suitable performa and will be
signed by him. While testing a homogenous control segment, any deflection reading
varying by more than 20% will be considered doubtful, and the test will be repeated.
The data will be examined by the Pavement Specialist in the light of pavement
condition/ composition data and tests giving incompatible results will be repeated.

G) Documentation
All the data sheets will be countersigned by the Highway Engineer and Pavement
Specialist properly filed and sent to the Office for data analysis.

H) Work Procedure
The working procedure as elaborated in IRC: 81-1997, for measuring the deflection of
the pavement is briefly described below:
 The point on the pavement to be tested is selected and marked at a distance of
0.9m from the edge of the pavement as it is already a two-lane road.
 The dual wheels of the truck are centered above the selected point and the probe
of the beam is inserted between the wheels at the selected point
 The locking pin from the beam is removed; legs are adjusted so that the plunger
of the beam is in contact with the stem of the dial gauge
 The dial gauge is set at approximately 1 cm and the initial reading is recorded
 The truck is slowly driven a distance of 2.7 m and stopped and the intermediate
reading is recorded
 The truck is further driven forward a further 9 m and the final reading is
recorded - Pavement temperature is recorded at least once every hour by
inserting thermometer in the standard hole and filling up the hole with glycerol.
 Tyre pressure will be checked at two or three hour intervals during the day and
adjusted to 5.6 kg/cm2.

Side by side with the deflection measurements, the condition of the pavement will
also be recorded. This will supplement the data on road condition survey already
collected.

The supervising Highway and Pavement Engineer will have a periodic check
particularly with regard to the following aspects:

 Correct positioning of the deflection beam


 Free operation of the dial gauge spindle
 Correct measurement of pavement temperature
 Consistency of deflection values for homogenous segments

2.3.6 Geotechnical Investigations and Sub-Soil Exploration


Sub-soil investigation will be carried out as per “IRC: 78-2014 Standard Specifications
and Code of Practice for Road Bridges” and the relevant IS Codes. The scheme for the
boring locations and the depth of boring will be prepared by the consultants. The
works will be carried through a sub-consultant.

A) Equipment
The boring, sampling and testing Equipment will be supplied by the sub-
consultants in accordance with the requirements of the relevant IS standards.

B) Applicable Standards/Codes
The latest versions of relevant Codes and Practices will be judiciously referred for both
laboratory / field tests, some of which are mentioned below:

 IS 1892
 IS 1498
 IS 4464
 IS 6926
 IS 2720: Part 1 to 41

C) Regulatory Requirements
The work will commence after informing PWD. It will be ensured that the boring work
does not cause any adverse environmental/ social impacts.

Proper safety measures will be taken for each boring location.

E) Data Verification
The Material cum Geo-technical Engineer will periodically visit the boring sites.
Engineer experienced in geotechnical investigations will be positioned at site for entire
duration of the boring work to monitor the field investigation.

The results of the sub-surface investigation as furnished by the sub-consultants will be


examined by the Materials cum Geotechnical Engineer for consistency and
compatibility before acceptance. Tests to be carried out on Soil Samples

The criteria for selecting tests are for the evaluation of the following:
 Bearing Capacity for Foundations
 Slope stability analysis for embankment (If required)

The Consultant proposes the following tests for the evaluation of the above.

F) Detailed Working Procedure


This investigation work will be carried out at Major/Minor Bridges, and ROB’s where
the existing structure is widened or a new structure is proposed. The scheme for the
boring locations and the depth of boring will be prepared by the consultants based on
the design requirement. To prevent heave and disturbance of the soil at the bottom of
borehole, the level of drilling fluid in the hole must be equal to or higher than the
elevation of the ground water at all times. This condition will be strictly observed in
formation of the fine sand or silt or in operations involving undisturbed sampling.

Records of boring shall be maintained in accordance with IRC: 78. The boring records
shall indicate borehole number, time and date of start and completion of boring, type
of boring and/ or drilling diameter of borehole at different depths, depth of casing,
existing ground level, 'N' values from standard penetration tests, number and type of
soil samples collected and the depths at which samples were taken and all other
relevant data. All boring records shall be incorporated in the soil investigation report.

i. Boring in Overburden Soil


Boring in overburden soil should be carried out in accordance with the provisions of
IS: 1892/ IRC: 78. The boreholes should have a minimum diameter of 150mm and be
suitably lined throughout.

The toe of the lining should at no time be more than 1.0m above the level to which
the material has been removed from the borehole. Before taking any undisturbed
sample or making any in situ test, the lining shall be carried down to the bottom of
the borehole.

No water shall be added while boring through cohesive soils and cohesion-less soil
above the water table. While boring through cohesion less soil below water table,
water level in the casing should always be maintained at or above the water table.

The cutting brought up by the auger, shell or the cutting shoe of the split-
spoon or undisturbed sampler shall be carefully examined and the soil description
duly recorded after performing field identification tests.

Ground water level for each borehole shall be checked during boring operation and
shall be recorded in boring.

ii. Method of Classification


The different classification for geotechnical investigations are usually based on the
field observation by an experienced engineer and accordingly boring/drilling
operations are taken up. The soil will be classified based on gradation and consistency
test results and also plasticity product as per IS 1498.

G) Documentation
All the data/documents received from the sub-consultants will be suitably
numbered for usage, storage and retrieval in future.

2.4 QAP for Preparation, Checking and Review of Design


Every deliverable which is to be submitted to the PWD as per the RFP will undergo

Similarly all the deliverables will undergo a design review for its completeness
and compliance with the requirement of statutory codes and scope as per RFP.

2.4.1 Quality Assurance Check


The objective of check is to reveal conditions in a project output or in its characteristics
which may give rise to non-conformities in the expected quality of the deliverables.
Check will be implemented in projects and will include checking of any documented
result prior to approval for use.

The TEAM LEADER has the overall responsibility for planning of check; implementation
of the check in co-operation with TEAM LEADER. The TL is responsible for requisition
and implementation of the check, treatment of the check result and final appraisal of
the check documentation. TL shall especially monitor that check of interfaces between
the affected disciplines and subtasks takes place. TL shall ensure that the selected
checkers are qualified. All documents / deliverables like calculation, designs,
drawings, reports etc., shall be checked before submitting to the Client.

Three levels of professional checks are being done for drawings and other documents.
Checks proposed for documents:
 Self-Check by Project Engineer
 Check by senior person designated by Project Manager / Team Leader
 Check by Project Manager

Checks proposed for drawings:

A) Checking of Reports
Author of a report shall take a printout of the self-checked report and shall and over
the printout to the checker (typically LE) for checking. The checker shall perform
checking by marking corrections in red in the report and will return to the author for
incorporation. Author shall incorporate all corrections, take a fresh print and
forward to the checker, who shall verify the incorporation of comments.

TL will compile inputs from different authors (if applicable), check the complete
document and provide comments. Originator shall incorporate the comments, take
printout as Master Copy and forward to TL along with the check prints.

B) Checking of Drawings
Every Drawing released to the Client shall go through the following check process:

 Self-check by the Draftsperson: The TL shall ensure that the self-check is carried
out.
 Check by Engineer: Drawing shall be Red & Yellow checked (by marking red for
all errors and yellow for all correct items) for every item in the drawing and the
stamp shall be signed. This shall be done for every check-print by the Project
Engineer.
 Check by TL: Will check the drawing for its completeness and adherence to
 QMS.

2.4.2 Quality Assurance Review


The objective of review is to ensure that a project meets the client’s requirements
and adheres to governing professional standards. It involves interdisciplinary and
coherent examination of results to evaluate its ability to meet agreed and implied
requirements and to identify problem areas, if any. Review can be used during the
entire project implementation and will usually include an initial review (preventive
review).

Review will be conducted on a single document, on a particular project stage or on the


entire project. The TEAM LEADER has the overall responsibility for planning and
supervision of the implementation of review. The Lead Expert is responsible for
requisition and implementation of the review, treatment of the review result and
final appraisal of the review documentation.

2.4.3 Templates for Drawings


Project specific title blocks and templates for drawings shall be prepared, checked and
forwarded to Client for prior approval.

2.5 QAP for Documentation


The objective of the procedure is to ensure that all pertinent information is available to
personnel when required, and changes are implemented with certainty and that
documents are handled responsibly during and after completion of a project. It is the
overall responsibility of the Team Leader to ensure adherence to this procedure in the
projects under the TL’s purview.

Proper documentation procedure must be adopted as per QMS which will aid in easy
data storage and data retrieval. TL shall ensure that both softcopy and hardcopy filing

systems are in place. The person having prepared any document is responsible for
the distribution and correct filing. After project closure, the TL shall send requests for
archival of both softcopy and hardcopy files. The TL shall define deadlines for the
destruction of hardcopy files.

2.6 Correspondence Reference Number


All correspondence documents for a project shall be uniquely identified by a
correspondence reference number. For one project, each correspondence shall have a
unique serial number irrespective of the correspondence type code. All other
documents for a project shall be uniquely identified by a document number. Revisions
of a document shall be uniquely identified by a revision number.
