Dear Sir,

Greetings! I am writing today in response to your office’s requirement asking me to explain why I have
not been able to comply with the requirements for the issuance of a Sanitary Clearance on time.

First, I would like to commend your office for its commitment to ensure compliance of business owners
of the requirements imposed by law for sanitation. This shows that our laws are not useless, and are
thus faithfully implemented by front liners such as the local government units.

While I admit that there had been delay in the compliance of the sanitary clearance requirements, I
would like to clarify that it has been inadvertent due to circumstances I cannot control.

My business, YTE Carinderia, became operational on January 3, 2018 and I have since applied for a
Mayor’s permit, along with all necessary permits and clearances imposed by the City. The City Health
Office inspected our premises sometime in April. I was not told what specific requirements to submit
except for the X-ray result of our personnel, Cesar Acierto. Mr Acierto, initially submitted his X-ray in
August which was rejected as it was not recent. During said time, he was also informed of the need for a
fecalysis. Unfortunately, being not familiar with medical terms, he mistakenly heard it as urinalysis. So
when he went back to your office to submit, it was once again rejected. He was only able to fully comply
with the new X-ray and the fecalysis in September, during which time I was informed that another
personnel was needed in our small carinderia.

I would like to reiterate that our carinderia is a small one, only open during lunch, where my husband
cooks, and I manage the finances while I manage my mother’s business next door. However, to comply
with the CHO’s requirement, I looked for Mr. Acierto’s companion. I hired Ms. Marvie Asuncion, and her
Fecalysis and X-ray results were submitted on the third week of November and on December 10,
respectively.

Having gone through the process with your office and after having gained knowledge of the steps for
compliance for the first time, I promise that this incident will not happen again and that I will be able to
secure our 2019 clearance before the deadline.
