AFFIDAVIT OF UNDERTAKING AND DECLARATION

OF NON-LIABILITY OF DR. JUNE G. CAMITAN

I, GERALD B. DEL ROSARIO, of legal age, Filipino, single and

presently residing at Brgy. Gatid, Santa Cruz, Laguna, after having
been sworn in accordance with law, hereby depose and state, that –

1. I personally know Dr. June G. Camitan of Calamba City. He was

the person who helped me find work to assist him in his medical
facility at Land Transportation Office (LTO), Calamba City Branch
Office, Laguna.

2. I declare under oath that Dr. June G. Camitan has no knowledge

of any wrong-doing or irregularity in the performance of his
medical duties at LTO, Calamba City Office, Laguna.

3. I further declare under oath that Dr. June G. Camitan has no

knowledge or participation in any controversy or complaint
which may be attributed to me by any interested person in
relation to my work with him at his medical facility at Land
Transportation Office (LTO), Calamba City Branch Office, Laguna.

4. I declare under oath and hereby undertake to clear, absolve or

exclude Dr. June G. Camitan from any liability whatsoever
concerning any incident, complaint, or claim against me in
connection with my previous performance of my work and
qualifications at his medical facility at the Land Transportation
Office (LTO), Calamba City Branch Office, Laguna.

IN WITNESS WHEREFORE, I hereto affixed my signature this 21 st
