AFFIDAVIT OF LEGITIMATION

WE, AAA and BBB, both of legal age, husband and wife, respectively, and
presently residing at Mariposa Village, BF Homes, Las Piñas, after having been duly
sworn to in accordance with law, depose and state that:

1. We are the biological parents of CCC who was born on 12 November 2015 as
evidenced by the attached copy of the child’s Birth Certificate marked as
Annex “A”;

2. At the time of his birth, we, the biological parents were not yet married;

3. We were married on 14 July 2017 in Las Piñas City, as evidenced by the

attached Marriage Certificate, marked as Annex “B”;

4. By virtue of our subsequent marriage, said child is now legitimated by

operation of law; and

5. We are executing this Affidavit to attest to the truth of the foregoing facts,
for purposes of complying with the requirements in applying for the
legitimation by subsequent marriage of our son, and for whatever legal
purpose it may serve.
