AFFIDAVIT OF WAIVER

I, ____________________, of legal age, single, Filipino and with residence and

having been duly sworn to in accordance with law, hereby depose and state to wit:

That I will undergo on the job training at General Santos City Airport, starting
June 2019.

That by this affidavit of waiver, I do hereby waive my rights to claim damages

and or other money claims in case of death or injury that may occur on my part while in
the course of my duty in the said place.

That this affidavit is executed to attest the truth of all the foregoing facts for
whatever legal purposes it may serve.
