NoE Form Filling Procedure

 Download and save the NoE form MSF 4274 on our computer. You can download the form
by clicking on the NoE Form link on the website www.springdaleindia.com. Alternatively, you
can go to Google and type MSF 4274 in the search box. You can download the form from the
Google search result.

 Take a printout of the NoE Form MSF 4274. Both coloured and black & white printouts are
acceptable. The total number of pages is 10.

 Please read the form in full. Page No. 6, 7 and 8 gives information on how to fill up the NoE
Form.

 Fill up the NoE form in CAPITAL LETTERS with BLACK PEN. A sample filled up NoE form is
uploaded on the website www.springdaleindia.com. You only need to fill Pages 1 – 5.

 Page No. 8 of the NoE form gives the address wherein you need to courier the documents.
As documents are being sent in ORIGINALS, it is strongly advised that you use a good courier
service like DHL. The cost of DHL courier service will be around 4,000 Rs/- approximately.

 The NoE processing time is normally 28 days excluding postage time (going and coming).
Sometimes, the NoE may come earlier than date whilst at times it may be delayed also. It
depends upon the existing work load at the MCA office.

 The list of documents that you need to send to the MCA for NoE are the following:
a. NoE Form – MSF 4274
b. Passport – 1st and last page notary (lawyer) attested copy
c. CDC – ORIGINALS ONLY
d. Sea Service Letter from Company or Masters Testimonials – ORIGINALS ONLY
e. 6 months of bridge watch-keeping certificate – ORIGINALS ONLY
f. STCW basic courses (PST + PSSR + FPFF + EFA) – ORIGINALS ONLY
g. Valid Medical Fitness Certificate (ILO 147 Format) from any DG Shipping approved
doctor – ORIGINALS ONLY
h. 2 passport size coloured photographs (40mmx50mm)
i. DD of 201 GBP in favour of “THE MARITIME AND COASTGUARD AGENCY” – Please
contact Mr. Rahul Mhatre from Cox & Kings at 09769388473 if you face any difficulty
in making the DD.

Important Points whilst filling the NoE Form:

Leave the SDS section blank. SDS is a reference number generated by the MCA when they receive
your application. Please do fill up the NAME and Date of Birth given at the extreme right hand corner
of the NoE Form.

The MCA will return your documents by courier. If the courier service is not good in your residential
area, please fill up an alternative address in some big city like Mumbai, Delhi wherein the MCA can
return your documents. Please mention this new address in the section on “Address for return of
documents”.

If you do not know the Official Number of any ship, leave that column blank.

Section 4A – Put a tick mark in all the five boxes.

Section 4B – Put a tick mark for all 4 STCW courses (PST + PSSR +FPFF + EFA)

Leave the first and last question of Section 4B blank as it is for deck cadets following the UK pre-sea
route.

Section 5 - In Declaration Box, put your signature and the date on which you have signed.

Section 6 – This section needs to be filled by a person who knows you in a professional capacity but
is not your friend or relative. Hence, it CANNOT be signed by your friends, parents or relatives. You
can have this section signed by your Captain, Chief Engineer, Chief Officer, Fleet Manager,
Superintendent, Doctor, Lawyer, Police Officer, Bank Manager, School / College teacher, etc.

You will most likely be making a payment by Demand Draft. Hence, you only need to put a tick mark
in the section Cheque / Bankers Draft.

If you are making payment by a credit card (debit cards are not accepted), you need to put your
credit card details along with your signature and date on this form.

Photographs – 2 photographs of 40mmx50mm size is required. The back of one photograph must
have your Name and Date of Birth written behind it. The back of the other photograph must have
your Name written on it and “I certify that this is a true likeness”. Then, it needs to be countersigned
by the same person who signed in Section 6 of the NoE form.
