AFFIDAVIT

I, ELDIE D. CAÑAS., Filipino of legal age, married and a resident of Balza, Alta
Tierra Subd., Napolan, Pagadian City, Zamboanga del Sur after having been duly
sworn to in accordance with law, do hereby depose and say that;

1. I am currently a Planning Officer II at the Provincial Planning and

Development Office, Zamboanga del Sur;

2. As per right of every employee, I have expressed my desire to retire

from government service and that I have already applied for the same;

3. While working, I have incurred financial obligations to meet financial

needs.

4. As such, I hereby authorize the proper department to deduct any

financial obligations that I have incurred while working.

5. That finally, I am executing this affidavit to attest to the truth of the

foregoing statements, to support my application to receive the said
benefits, and for any legal purposes this may serve;
